Imipenem-amikacin as initial or secondary treatment in febrile neutropenic patients

Özet

Febril nötropenik hastalarda imipenem (2 g/gün) amikasin (1-1.5 g/gün) kombinasyonunun klinik etkinliği başlangıç tedavisi (Grup 1) ve ikincil tedavi (Grup 2) olarak araştırılmıştır. Birinci grupta 40 febril atak (33 hastada), 2. grupta 34 febril atak (31 hastada) yer almıştır. Nötrofil sayısı 38.5°C veya bir günde iki kez >38.0°C vücut ısısı saptanması halinde antibakteriyel tedavi başlanmıştır. Birinci grupta 24/40 (%60); 2. grupta 20/34 (%58.8) atak tam yanıt sağlanmıştır. Hastaların hiçbirisinde tedavi kesilmesini gerektirecek ciddi yan etki gözlenmemiştir, izole edilen 14 etkenin sekizini gram-pozitifler, altısını gram-negatif bakteriler oluşturmuştur. Üç Pseudomonas aeruginosa izolatı da dahil olmak üzere tam bakteriler imipenem ve amikasine duyarlı bulunmuştur. Bu sonuçlar, imipenem-amikasin rejiminin febril nötropenik hastalar için uygun bir tedavi seçeneği olduğunu düşündürmektedir.

Abstract

Clinical efficacy of imipenem (2 g/day) combined with amikacin (1-1.5 g/day) as initial (Group 1) or secondary (Group 2) treatment in febrile neutropenic patients was evaluated in this study. Forty febrile episodes (in 33 patients) were evaluable in Group 1 and 34 febrile episodes (in 31 patients) in Group 2. All of the patients received prophylactic trimethoprim-sulfamethoxasole, fluconazole and acyclovir at a neutrophil count of 38.5°C lasting for at least an hour or >38.0°C twice in a day. A prompt response was observed in 24/40 (60%) febrile episodes in Group 1 and 20/34 (58.8%) in Group 2. None of the patients experienced severe side effects to require cessation of treatment. Moreover none of the infecting microorganisms isolated (14 isolates in total; eight gram-positives with no methicillin-resistant staphylococci; and six gram-negatives with three isolates of Pseudomonas aeruginosa) were resistant to either antibiotics. These results suggest that imipenem-amikacin is an effective regimen for febrile neutropenic patients.
