Özet

Çalışmada Hepatit B virüs (HBV) belirleyicileri ile HBV DNA'nın karşılaştırılması amaçlanmıştır. Hepatit B serolojik belirleyicileri değişik profillerde olan 247 hasta serum örneğinde kemiluminisant moleküler hibridizasyon yöntemi ile HBV DNA araştırılmıştır. Hastaların hepatit B belirleyicileri mikroenzim-immun-assay yöntemi ile incelenmiştir. HBsAg, anti-HBc ve HBeAg olumlu 45 kronik hepatit olgusunun 36'sında (% 80.0); HBsAg, anti-HBc ve anti-HBe olumlu 148 kronik hepatit olgusunun ise 18'inde (% 12.16) HBV DNA olumlu olarak bulunmuştur. Her iki grup arasında HBV DNA olumluluğu ve türeleri açısından anlamlı fark saptanmıştır (p=0.00001 ve p=0.00003). HBsAg, anti-HBc total yanında HBeAg ve anti-HBe'nin birlikte olumlu olduğu dört olgunun ikisinde, aynı tabloda son iki belirleyicinin birlikte olumsuz olduğu iki olgunun ikisinde de HBV DNA 10 pg/ml'nin üzerinde elde edilmiştir. Tüm belirleyicileri olumsuz olan 10 olgunun, tek başına anti-HBc antikoru olumlu olan 15 olgunun, yalnızca anti-HBc ve anti-HBe olumlu olan 14 olgunun hiç birinde HBV DNA 10 pg/ml'nin üzerinde saptanmamıştır. Tek başına HBsAg olumlu olan yedi olgunun birinde, yalnızca anti-HBc total ve HBeAg olumlu olan iki olgunun birinde HBV DNA 10 pg/ml üzerinde olumlu olarak elde edilmiştir. Serum HBV DNA 'sının HBeAg olumluluğu ile paralellik gösterdiği; HBV DNA'nın kantitatif araştırılmasının viral replikasyonun değerlendirilmesi ve farklı serolojik profillerin yorumlanmasında yol gösterici olduğu sonucuna varılmıştır.

Abstract

The aim of this study was to compare Hepatitis B Virus (HBV) serologic markers with HBV DNA. Hepatitis B Virus DNA was investigated in the sera of 247 patients with different hepatitis B serological markers by chemiluminescent molecular hybridization assay. Patients' hepatitis B markers were analyzed by microenzyme immun assay. Hepatitis B Virus DNA was detected in 36 of 45 (80.0 %) chronic hepatitis B cases with positive HBsAg, anti-HBc and HbeAg and in 18 of 148 (12.16 %) chronic hepatitis cases with positive HBsAg, anti-HBc and anti-HBe. A statistical difference existed between the HBV DNA positivity rates and titers of two groups. Among the four cases with positive HBsAg, anti-HBc,HBeAg and anti-HBe, two had titers of HBV DNA over 10 pg/ml. HBV DNA was found to be over 10 pg/ml in two other cases having HBsAg and anti-HBc without HBeAg and anti-HBe. On the there side, HBV DNA was detected to be negative or lower than 10 pg/ml in 10 cases with negative hepatitis B markers, in 15 cases with positive anti-HBc as the only marker and in 14 cases with positive anti-HBc and anti-HBe. Hepatitis B Virus DNA was found to be over 10 pg/ml in one of seven (14.29 %) cases with HBsAg as the only marker and in one of two (50.0 %) cases with positive anti-HBc and HBeAg. As a result; it was concluded that serum HBV DNA was parallel to the existence of HBeAg, and the quantitative detection of HBV DNA is useful for the assessment of viral replication and.
