Mycobacterium tuberculosis ve Nocardia brasiliensis' in birlikte etken oldukları bir psoas apsesi olgusu

Özet
Psoas apselerinden nadir bazı etkenlerin izole edilebileceği, bazen birden fazla etkenin sorumlu olabileceği, bu nedenlerle de mikrobiyolojik incelemelerde gösterilecek titizliğin önemi açısından bu olgu sunuldu. Yirmi iki yaşındaki erkek hasta bir aydır gece terlemesi, 15 gündür bel ağrısı ve sağ kasığında kitle nedenleriyle hastaneye başvurdu. Karın ultrasonografisi ile psoas apsesi tanısı konuldu. Bilgisayarlı tomografi eşliğinde drene edilen apse materyalinin Gram boyamasında bol polimorf çekirdekli lökositler ve granüllü boyanan, dallanmış Gram-pozitif basiller dikkati çekti. Modifiye Ziehl-Neelsen (ZN) boyamada yine dallanma gösteren zayıf aside dirençli basiller, ayrıca ZN boyamada aside dirençli basiller görüldü. Kültürlerde Nocardia brasiliensis ve Mycobacterium tuberculosis izole edildi. Apsesi drene edilen ve Pott hastalığı nedeniyle korpektomi uygulanan olguya, anti-tüberkülo tedavi yanında, nokardiyoza yönelik olarak 15 gün süreyle 1 g/gün amikasın ve üç ay süreyle 3x1 trimetoprim-sülfametoksazol fort tablet uygulandı. Tedavinin beşinci ayında yapılan kontrollerinde herhangi bir patoloji saptanmadı. Anti-tüberkülo tedaviyi bir yıla tamamlamak üzere kontrollerini yaşadığı ilde sürdürmesine izin verildi.
Abstract
This case is presented to show the importance of pedantry during microbiological examinations as some rarely seen microorganisms may be isolated and sometimes more than one microorganism may be responsible for psoas abscesses. Twenty-two year-old male patient admitted to the hospital with complaints of night sweats for one month, back pain and right inguinal mass since 15 days. Psoas abscess was observed in abdominal ultrasound. In the Gram stain of abscess material that had been drained with the aid of computed tomography numerous polymorphonuclear leucocytes and branching beaded, filamentous Gram-positive bacteria were seen. Weak acid-fast branching bacteria, and acid-fast bacteria were seen after staining with modified Ziehl-Neelsen (ZN) method. Nocardia brasiliensis and Mycobacterium tuberculosis were isolated in the cultures. Besides antituberculous therapy, 1 g/day amikacin for 15 days and 3x1 trimethoprim-sulfamethoxazole forte tablets against nocardiosis were given to the patient who also abscess drainage and corpectomy operations because of Pott' s disease. No pathology was found during the controls in the fifth month. He was let to continue the controls at his home and continue antituberculous therapy for twelfth months.
