Özet

İmmun deprese çocuklarda mantar infeksiyonlarının tedavisinde lipozomal amfoterisin B'nin etkisi saptamak amacı ile bu çalışma yapıldı. İstanbul Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Hematoloji-Onkoloji Ünitesi'nde kemoterapi gören ve medyan yaşı 8.5 (2,5-17) olan 18 hastaya (13 akut lösemi, dört lenfoma, bir indifferansiye sarkom ve olguların altısı relaps yapmış) nötropenik dönemde klinik, radyolojik ve/veya mikrobiyolojik bulgularla mantar infeksiyonu tanısı konuldu. Yetmişiki saat sonunda antibiyotiklere yanıt vermeyen ateş olası mantar infeksiyonu kabul edilerek 1-4 mg/kg/gün dozlarda Lipozomal Amfoterisin B (Lip A) uygulandı. Yirmi bir epizodun 10 'unda etken izole edildi (dört Aspergillus türü, altı maya cinsi mantar). Tutulan bölge en sık akciğer, paranazal sinüsler ve sindirim sistemi mukozası idi. Daha az sıklıkla septisemi görüldü. Etkenin izole edildiği epizotların 6/10 'unda; olası mantar infeksiyonu dan epizotların 8/11 'inde klinik ve radyolojik iyileşme sağlandı, infeksiyonların iyileşmesi nötropeniden çıkışla paraleldi. Diğer hastalar yanıtsız kaldı veya kemik iliğinin remisyona girmemesi, hepatotoksisite, Gram-negatif süperinfeksiyon gibi nedenlerle kaybedildi. En sık görülen yan etki nefrotoksisite (replasman ile düzeltilebilen asemptomatik hipopotasemi) idi. Bir hastada reversibl üremi görülürken bir hastada uzun süre yüksek doz Lip A kullanımı sonrası tedaviyi kesmeyi gerektiren hepatotoksisite/serum transaminazlarında dört kat artış ve hiperbilirubinemi gelişti. Sonuç olarak, Lip A immundeprese hastalarda iyi tolere edilebilen etkin bir ilaçtır. Ancak tedavi maliyeti kullanımını kısıtlaya-bilmektedir.

Abstract

This study was carried out in order to determine the efficacy of lipozomal amphotericin B in the treatment of mycoses in immunodepressed children. In the Pediatric Hematology-Oncology Department of Istanbul Faculty of Medicine, 18 patients (13 acute leukemia, four lymphoma, one undifferentiated sarcoma) with a median age of 8.5 years (2.5-17 years) were treated for documented or assumed fungal infections. A neutropenic fever persisting longer than 72 hours and unresponsive to antibiotics was also assumed as fungal infection and Liposomal Amphotericin B (Lip A) was started at a dose of 1-4 mg/kg/day. Out of twenty one episodes 10 were documented (four Aspergillus spp., six Candida spp.). The most frequently affected organs were lungs, paranasal sinuses and mucous membranes of the gastrointestinal tract. Fungal septicemia was also detected in two patients. In 6/10 of documented episodes and 8/11 of assumed episodes, outcomes were clinical and radiological cure which were associated with recovery from neutropenia. Remaining patients were either unresponsive to Lip A or died with resistant malignancy, hepatotoxicity or Gram-negative superinfections. The most common adverse effect of Lip A was nephmtoxicity presenting as asymptomatic hipopotassemia and controlled with i.v. potassium replacement. One patient had reversible uremia after long term Lip A application and another patient had hepatotoxicity with hyperbilirubinemia and four fold increase in transaminase levels. In conclusion, Lip A is well tolerated and effective in immunocompromised children, but high cost may sometimes restrict its application.
