Birinci basamakta çalışan sağlık personelinde viral hepatit A, B, C ve E seroprevalansı ve personelin bu infeksiyonların bulaşma yollarını bilme durumu

Özet
Ekim 1996'da yapılan bu kesitsel araştırma da, Gölbaşı Eğitim Araştırma Sağlık Grup Başkanlığı Bölgesi'nde (birinci basamak sağlık hizmetlerinde) çalışan sağlık personelinin hepatit A, B, C, E infeksiyonları için immünolojik değerlendirilmesi ve bu personelin enfeksiyonların bulaşma yollarını bilip bilmediklerini ortaya koymak amaçlanmıştır. Doktor, hemşire ve/veya ebeleri içeren toplam 60 personelin tümüne ulaşılmış ve hepatit markerları (HBsAg, anti-HBs IgG, anti-HBs IgM, anti-HBc total, HBeAg, anti-HBe, anti-HAV total, anti-HCV, anti-HEV) ELISA yöntemi ile araştırılmıştır. Altmış sağlık personelinin % 90'ında anti-HAV Ig total, % 6.7'sinde HBsAg, % 51.7'sinde anti-HBs IgG, % 3.3'ünde anti-HBs IgM pozitif bulunmuştur. Hepatit B' ye karşı % 28.3'ü infeksiyonu geçirerek bağışıklık oluşturmuşken, % 23.4'ü hepatit B infeksiyonuna karşı aşılanmıştı. Personelin % 1.7'sinde anti-HCV pozitif olarak saptanmıştır. Anti-HEV hiçbir personelde görülmemiştir. Altmış sağlık personelinin % 55.0'i hepatit A infeksiyonunun bulaşma yollarını doğru olarak belirtmiştir. Bu sayı hepatit B infeksiyonu için % 73.3, hepatit C için % 53.3 ve hepatit E için % 15.0 olarak saptanmıştır. Birinci basamakta çalışan sağlık personelinin düzenli aralıklarla viral hepatitler konusunda eğitilmesi gerektiği sonucuna varılmıştır.
Abstract
This cross-sectional study, which was carried out in October 1996, aimed at the immunologic assessment of hepatitis A, B, C and E infections in health personnel working in primary health care services in Gölbaşı Education and Research Region and determination of their degree of knowledge on the transmission routes of these infections. All 60 personnel, including doctors, nurses and midwives were examined for the presence of markers HBsAg, anti-HBs IgG, anti-HBs IgM, anti-HBc total, HBeAg, anti-HBe, anti-HAV total, anti-HCV, anti-HEV using ELISA. The results were as follows: 90 % anti-HAV Ig total (+), 6.7% HBsAg (+), 51.7 % anti-HBs IgG (+), 3.3 % anti-HBs IgM (+), 28.3 % had had hepatitis B infection-immune, 23.4 % vaccinated for hepatitis B, 1.7 % anti-HCV (+), and none positive for anti-HEV. Of the personnel 55.0 % knew correctly the transmission routes of hepatitis A. Correct knowledge was 73.3 % for hepatitis B, 53.3% for hepatitis C, and 15.0 % for hepatitis E. It is concluded that primary health care personnel should be regularly trained on viral hepatitis.
