Üropatojen Escherichia coli suşlarının aerobaktin oluşturma özelliği ve fimbriyalarının incelenmesi

İdrar yolu infeksiyonlu hastalardan izole edilen 115 Escherichia coli suşu ile sağlıklı çocukların dışkı örneklerinden normal flora bakterisi olarak izole edilen 30 kontrol E. coli suşu aerobaktin oluşturma ve fimbriyaları yönünden incelenmiştir. Üropatojen E. coli suşlarında ve kontrol suşlarda aerobaktin üretenlerin oranı sırasıyla % 42.6 ve % 43.3 olarak saptanmıştır. Ancak idrar yolu infeksiyonlarının farklı klinik tabloları dikkate alındığında; piyelonefritli erişkin ve çocuk hastalardan izole edilen suşların sırasıyla % 64.3 ve % 83.3'ünün aerobaktin ürettiği belirlenmiştir. İdrar yolu infeksiyonu için predispozan faktörleri taşıyan hasta izolatlannda aerobaktin üretenlerin oranı (% 27.3), bu faktörleri taşımayan hasta izolatlarındakinden (% 44.2) daha düşük bulunmuştur. Aerobaktin ve MR fimbriyaya birlikte sahip olan suşlara en yüksek oranda piyelonefrit etkenleri (% 21.4) ve en düşük oranda dışkı izolatları arasında (% 3.3) rastlanmıştır (0.10>p>0.05). İdrar yolu infeksiyonlarının tüm farklı klinik tabloları dikkate alındığında; etken E. Coli suşları içinde aerobaktin oluşturan ve MR fimbriya taşıyanların sıklığı (% 11.6-21.4), aerobaktin oluşturan ve MS fimbriya taşıyanlarınkine (% 6.7-8.4) göre daha yüksek saptanmıştır.
Abstract
A hundred-fifteen Escherichia coli strains isolated from patients with urinary tract infection and 30 control E. Coli strains isolated from the stool samples of healthy children as normal flora were examined from the viewpoint of aerobactin production and fimbrial characteristics. The percentage of aerobactin producing strains in uropathogen E. Coli strains and control strains were determined as 42.6% and 43.3%, respectively. However, when the lower and upper urinary tract infections were taken into consideration separately, it was found that the strains isolated from adult and pediatric patients with pyelonephritis produced aerobactin 64.3 % and 83.3 %, respectively. The percentage of aero-bactin producing isolates from patients with predisposing factors for urinary tract infections were found as 27.3 %, which is lower when compared with patient isolates which do not have these factors (44.2 %). Strains which possess aerobactin and MR fimbriae were found in highest ratio among pyelonephritis isolates (21.4%) and in lowest ratio in stool isolates (3.3%) (0.10>p>0.05). When all of the different clinical categories of the urinary tract infections were considered, it was found that among the uropathogen E. coli strains, those which produced aerobactin and carried MR fimbriae had a higher frequency of 11.6-21.4% compared to those which produced aerobactin and carried MS fimbriae which had a frequency of 6.7-8.4 %.
