Özet

Hastane personelinin elleri hastane infeksiyonlarında önemli rol oynarken, eller nedeni ile steteskop kirlilikleri de önemli olabilmektedir. Patojen mikroorganizmanın hastadan çevreye ve hastadan hastaya bulaşı sıklıkla ellerden başlamaktadır. Bu çalışmanın amacı ellerin ve steteskopların bulaş kaynağı olabilme özelliğini araştırmaktır. Çalışmada, İzmir Göğüs Hastalıkları ve Cerrahisi Eğitim ve Araştırma Hastanesi'nde hastane çalışanlarının ellerinden ve steteskoplarından önceden haber vermeden ve el ve steteskoplardan simültane ilk sürüntü örnekleri, steteskoplarda ve ellerde yapılan dezenfeksiyonun ardından tekrar ikinci sürüntü örnekleri alınmıştır. Hem ellerde hem de steteskoplarda dezenfeksiyon öncesi ve sonrası baskın bakterilerin identifikasyonu ve koloni sayıları (CFU) yapılmıştır. Bu arada farklı dezenfeksiyonların (klorheksidin, %70'lik alkol, benzalkonium klorid) etkisi gözlemlenmiştir. Çalışmada %91.85 ellerde, %57.76 steteskoplarda patojen mikroorganizmalar izole edilmiştir. Ayrıca patojen mikroorganizma izole edilen steteskoplardan %65.9 olguda aynı etkenler hem elden hem de steteskoplardan izole edilmiştir. Benzer mikroorganizmalara ilişkin olarak ellerde steteskoplara göre daha çok sayıda CFU sayılmıştır. Sonuç olarak, ellerin primer ve sekonder bulaş kaynağı olarak önemli olduğu, ancak kısa süreli bile olsa dezenfeksiyonun ortamdan bakterileri eradike etmesi yönünden önerilebileceği gösterilmiştir.

Abstract

The hands of hospital staff are a major problem in hospital infections and the stethescopes may be contagious due to the hands of health care workers. The spread of pathogens from the environment to patients and from patient to patient frequently begins with the hands. This study was carried out to find out the effect of hands and stethescopes as well in nosocomial infections. The study was performed in the Izmir Chest Diseases and Training Hospital, from January to April 2000, and 64 of the personnel were, without preannouncement, investigated for pathogen microorganisms in their hands and stetescopes simultaneously. Hand cultures were taken from palms and fingers of both hands, and stethescope cultures were taken with sterile swabs. At the same time, the effect of disinfectants was investigated. The cultures were incubated in aerobic conditions. The efficacy of some disinfectants such as chlorhexidine, alcohol %70 and benzalkonium chloride were investigated. Pathogenic microorganisms were isolated in rates of 91.85 % and 57.76 % from the hands and stethescopes respectively. Of the pathogens 65.9 % isolated from the stetescopes were completely similiar to the hands of the health care workers, but differed in number (CFU). In conclusion, hands are important as a primary and secodary contagious factor and desenfection is advisable for eradication of bacteria.
