Özet

Bu çalışmada, gebe ve yenidoğan kordonundan alınan kanlarda tetanoz antitoksini ve buna etkili faktörlerin araştırılması amaçlandı. Çalışmaya 88 gebe ve bebekleri alındı ve yöntem olarak ELISA uygulandı. Gebelerin %73.8'inde (65/88) tetanoz antitoksini (TAT) pozitif bulundu. Bu annelerin bebeklerinde de TAT pozitif idi. Yirmi-bir yaşın altında TAT pozitifliği %93.7, 21-30 yaş arasında %75 ve 30 üstünde ise %41.6 olarak saptandı. Anne yaşının artmasıyla TAT seropozitifliğinin azalmasının primer aşılamadan sonraki rapellerin düzensiz uygulanmasına bağlı olabileceği düşünüldü. Annenin yaşadığı bölgenin, doğum sayısının, kronik hastalığının olup olmamasının ye bebeğin doğum ağırlığının TAT seropozitifliğine etkisinin olmadığı görüldü. Yüksek ölüm oranlarına neden olan yenidoğan tetanozunun önlenmesi için gebelerin aşılanması, primer aşılamadan sonra rapel dozların uygulanması, doğum sırasında hijyen kurallarına uyulması ve TAT seropozitifliği ile tetanoz aşısının etkinliğinin kontrolü önemli yaklaşımlardır.

Abstract

The purpose of the present study was to determine the protection against tetanus in pregnants as well as in their newborns. The tetanus antitoxin levels in maternal and cord sera were detected by ELISA and factors affecting the antitoxin levels were investigated. Eighty-eighty mothers and their babies were included in the study. Tetanus antitoxin (TAT) seropositivity was found in 73.8 % (65/88) of 88 mothers. The babies of those mothers were also seropositive. TAT seropositivity was found as 93.7 % (15/16) in the group under 21 years of age, 75 % (45/60) in the 21-30 age group and 41.6 % (5/12) in over 30 years of age group. The reason of decreased seropositivity ratios with increasing mother age was thought to be related to irregular application of booster doses after primary immunization. The factors such as the region where the pregnant women lived (whether urban or suburban), number of labors, chronic disease of mothers and birth weights of newborns were found to have no effect on TAT seropositivity in the mothers and their babies. It is concluded that vaccination of pregnants against tetanus, application of booster doses after primary immunization, hygienic rules during labor and control of the efficacy of tetanus vaccine with TAT seropositivity are of utmost importance in protection against neonatal tetanus
