Yeni kuşak ß-laktam antibiyotiklere dirençli Pseudomonas suşlarında bulunan ß-laktamazların "isoelectric focusing" yöntemi ile tiplendirilmesi

Yeni kuşak beta-laktam antibiyotiklere dirençli 38 Pseudomonas suşunda beta-laktamaz varlığını araştırmak bu çalışmanın amacı idi. beta-laktamazlar, "isoelectric focusing" yöntemi ile izoelektrik noktaları belirlenerek ve beta-laktam antibiyotiklere duyarlılıkları incelenerek tiplendirildi. Çalışma grubunu oluşturan Pseudomonas suşlarından 37'si P. aeruginosa, biri Burkholderia cepacia idi. Özgül antiserumlanyla tiplendirilen P. aeruginosa suşlarından dokuzunun O11, sekizinin II, beşinin O4, ikisinin O12 serotipinde olduğu belirlendi ve 13 suş tiplendirilemedi. Çalışmaya alınan suşların hepsinde beta-laktamazların sentezlendiği gösterildi. "Isoelectric focusing" yöntemi ile bazı suşlarda enzim bantları gözlenemezken, bazılarında bir, iki veya daha fazla enzim bantı olduğu görüldü. Bu enzim bantlarının izoelektrik noktaları 7.6 olarak gruplandırıldı. Ayrıca Pseudomonas suşlarının beta-laktam antibiyotikler için MIK değerleri belirlendi. Pseudomonas'ların antibiyotiklere karşı in vitro olarak etkilerine ve içerdikleri enzimlerin izoelektrik noktalarına göre beta-laktamazlar gruplandırıldı. Pseudomonas suşlarından yedisinin grup l, 7'sinin grup 2be, birinin grup 2c, 11 'inin grup 2d ve beşinin grup 3 beta-laktamaz içerdiği görüldü. Klinik örneklerden izole edilen yeni kuşak sefalosporinlere dirençli Pseudomonas Varda bulunan beta-laktamaz tiplerinin bilinmesi tedavide uygulanacak beta-laktam antibiyotiklerin seçiminde yol gösterici olacaktır.
Abstract
The aim of this study was to investigate the production of ft-lactamase of 38 Pseudomonas isolates resistant to newer beta-lactam antibiotics. The beta-lactamases were typed according to isoelectric point values of beta-lactamases determined by "isoelectric focusing" and susceptibility to fi-lactam antibiotics of Pseudomonas isolates. Thirty-seven of the isolates were P. aeruginosa and one was Burkholderia cepacia. Pseudomonas aeruginosa isolates were serotyped with available antisera. The serotypes of nine P. aeruginosa isolates were O11, eight were II, five were O4 and two were O12. The serotypes of 13 isolates were non-typable. beta-lactamase expression was detected in all of Pseudomonas isolates. It was observed that there was no enzyme band in some isolates with "isoelectric focusing" although one, two or more enzyme bands were seen in some isolates. The isoelectric points of enzyme bands were grouped as 7.6. MIC values to beta-lactam antibiotics of Pseudomonas isolates were investigated. The ft-lactamases were grouped according to in vitro susceptibility of Pseudomonas isolates to antibiotics and the isoelectric points of the enzymes. It was suggested that seven of the isolates had group 1, seven group 2be, one group 2c, 11 group 2d and five group 3 enzymes. The characterization of beta-lactamase patterns of Pseudomonas isolates resistant to new generation cephalosporins will be useful for the right choice of beta-lactam antibiotics that will be applied for treatment.
