Akut viral hepatite bağlı fulminan karaciğer yetmezliği gelişen olguların değerlendirilmesi

Özet
Bu çalışmanın amacı, akut viral hepatite bağlı fulminan karaciğer yetmezliğinin klinik özelliklerini değerlendirmek idi. Akut viral hepatite bağlı fulminan karaciğer yetmezliği (FKY) gelişen dokuz olgu retrospektif olarak incelendi; klinik ve laboratuvar bulguları, tedavi ve prognozları yönünden değerlendirildi. Olgularda yaş, etiyoloji, ansefalopati öncesi sarılığın süresi, başvuru anındaki klinik durumları, protrombin zamanı (PTZ) ve bilirubin düzeyi gibi prognoza etki eden faktörler karşılaştırıldı. Hastaların yaş ortalaması 35.4 (15-75) idi. Olguların iki tanesi viral hepatit A (VHA), dört tanesi viral hepatit B (VHB), üç tanesi viral hepatit B+D (VHB+D) olarak saptandı. Viral hepatit A'lı iki olgudan bir tanesi, VHB'li dört olgudan üç tanesi, VHB+D'Iİ üç olgudan bir tanesi kaybedildi. İyileşen dört olgunun hepsi 45 yaşın altında olup ansefalopati evresi 1 ve 2 olarak belirlendi. Tüm olgularda PTZ 18 sn'den uzun iken kaybedilen olguların hepsinde 50 sn'nin üzerinde idi. Hepatik koma tedavisi yanında, bir hastaya exchange transfüzyon ve bir hastaya da plasmaferez uygulandı. Sonuç olarak, FKY 'ne gidiş VHB olan olgularda daha yüksek bulundu. Uzamış PTZ'nin fulminansa gidiş ve prognoz açısından önemli olduğu gözlendi. Olgularda, exchange transfüzyon ve plasmaferezin etkili olmadığı gözlendi.
Abstract
The aim of this study was to evaluate the clinical picture in fulminant hepatic failure following acute viral hepatitis. Nine cases with fulminant hepatic failure (FHF) due to acute viral hepatitis hospitalized at the Department of Infectious Diseases and Clinical Microbiology of Izmir SSK Tepecik Teaching Hospital between January 1995 and June 1998, were retrospectively studied. The cases were compared as to factors such as age, etiology, duration of jaundice until onset of encephalopathy, clinical condition at the time of presentation, prothrombin time (PTZ) and serum bilirubin level. The range of age was 35.4 (15-75). Two of the cases were viral hepatitis A (VHA), four were viral hepatitis B (VHB) and three were viral hepatitis B+D (VHB+D). One of the VHA, three of the VHB and one of the VHB+D cases died. All of the recovered four cases were under the age of 45 and encephalopathy grades were 1 or 2. The prothrombin time was longer than 18 seconds in all of the cases, however, the PTZ of all lost cases were longer than 50 seconds. In addition to the treatment of hepatic failure, one case received exchange transfusion and another plasmapheresis. In conclusion, passing to FHF was found to be more frequent in VHB than in the others. Prolonged PTZ was an important prognostic laboratory finding for passing to fulminant hepatitis. Exchange transfusion and plasmapheresis were observed to be ineffective in the treatment.
