Özet

Bu çalışma ile Manisa ve çevresindeki onikomikoz epidemiyolojisinin aydınlatılması amaçlanmıştır. Şubat 1995-Haziran 1999 tarihleri arasında Celal Bayar Üniversitesi Tıp Fakültesi'nde klinik olarak onikomikoz ön tanısı almış 990 olgu incelemeye alınmıştır. Hastaların tırnak kazıntı örnekleri mikolojik yöntemlerle değerlendirilmiş, incelenen örneklerin 315'inde (% 31.8) hem direkt bakı hem de kültür negatifken, 362'sinde (%36.6) direkt bakı pozitif, kültür negatif olarak bulunmuştur. Örneklerin 313'ünde (%31.6) kültürde üreme saptanmış, bunların ise 294'ünün (%94) direkt bakılarının olumlu olduğu gözlenmiştir. Kültürde üreme saptanan 313 olgunun 269'unda (%86) dermatofit, 44'ünde (%14) ise maya türü mantarlar üremiştir. İzole edilen dermatofitler sırası ile Trichophyton rubrum (252), T. mentagrophytes (14), Epidermophyton floccosum (2) ve T. tonsurans (1); izole edilen maya türü mantarlar ise sırası ile Candida tropicalis (13), C. albicans (11), C. plabrata (9), Trichosporon spp. (7), C. krusei (2), C. parapsilosis (1) ve Rhodotorula spp. (1) olmuştur. El tırnaklarında maya türü mantarların, ayak tırnaklarında ise dermatofit türü mantarların daha sık etken olduğu gözlenmiştir. Sonuç olarak; çalışma bulgularının genel olarak diğer çalışmaların bulgularıyla uyumlu olduğu görülmüş, Manisa ve çevresindeki onikomikoz etkenleri konusunda diğer araştırmacılar için kaynak oluşturabileceği düşünülmüştür.

Abstract

The aim of this study was to clarify the onychomycosis epidemiology in Manisa and its environment. Totally 990 cases who had been prediagnosed as onychomycosis at Celal Bayar University Hospital between February 1995 and June 1999 were investigated. The nail scrapings of the patients were evaluated with mycological methods: Both the direct microscopic examination and culture were negative in 315 samples (31.8%) whereas the direct microscopy was positive and culture was negative in 362 (36.6%). Fungal culture was positive in 313 samples and the direct microscopy was positive in 294 (94%) of these samples. Dermatophytes were isolated from in 269 cases out of 313 (86%) and yeasts from 44 (14%). The isolated dermatophytes were as follows: Trichophyton rubrum (252), T mentagrophytes (14), Epidermophyton floccosum (2) and T. tonsurans (1). The isolated yeasts were Candida tropicalis (13), C. albicans (11), C. glabrata (9), Trichosporon spp. (7), C. parapsilosis (1) and Rhodotorula spp. (1). It was seen that yeasts were more frequent in hand nails; on the other hand, dermatophytes were more frequent than yeasts in the toe nails. As a result, the findings of this study were generally similar to the findings of other studies. This study can be a source of information for other researches on onychomycosis in Manisa and its environment.
