Hastane infeksiyonlarından izole edilen Klebsiella pneumoniae ve Escherichia coli izolatlarında genişlemiş spektrumlu beta-laktamaz sıklığının saptanması ve izoelektrik fokuslama yöntemi ile tiplendirilmesi

Özet
Genişlemiş spektrumlu beta-laktamaz (GSBL) tipi direnç, özellikle Klebsiella pneumonias ve Escherichia coli'lerin etken olduğu hastane infeksiyonlarının tedavisinde önemli bir problemdir. Bu çalışmada; hastane infeksiyonu etkeni olan 143 E.coli ve 47 Klebsiella spp. izolatında GSBL sıklığı "çift disk sinerji yöntemi (Double Disk Synergy Test: DDST)" ile, beta-laktamaz tipleri ise "izoelektrik fokuslama (Isoelectric Focusing: IEF)" yöntemi ile araştırıldı. Escherichia coli ve Klebsiella spp. izolatlarında GSBL pozitifliği sırası ile %42.6 ve %3.1 olarak saptandı, izoelektrik fokuslama yöntemi ile GSBL pozitif izolatların %92'sinde pl'sı 7.6 olan, SHV-1 'e veya GSBL özelliği taşıyan SHV-2'ye ait bir bant bulundu. İzolatların %56'sında TEM-1 enzimi ile uyumlu pl'sı 5.4 olan bant görüldü. Üçüncü sıklıkla %24 oranında rastlanan ve SHV-3 ile uyumlu pl'sı 7.0 olan bant gözlendi. Çalışma sonuçlarına göre, SHV kökenli enzimler ile TEM-1 enziminin ilk sırada yer aldığı belirlendi. Sonuç olarak; beta-laktamazların pl değerleri ile antibiyotik duyarlılıklarının birlikte değerlendirilmesinin; direnç değişikliklerinin izlenmesi, direnç mekanizmalarının ortaya konması ve antibiyotik kullanım politikasının oluşturulmasında yararlı, ayrıca epidemiyolojik çalışmalar için de yardımcı olabilecektir.
Abstract
Extended spectrum beta-lactamase (ESBL) type of resistance is an important problem in the treatment of nosocomial infections especially where Klebsiella pneumoniae and Escherichia coli are the agents of infection. In this study, in 143 E. coli and 47 K. pneumoniae strains, which are nosocomial infection agents, ESBL production was determined with the Double Disc Synergy Test (DDST) and the frequency of the ESBL were found 3.1% and 42.6%, respectively. Then, beta-lactamase types were estimated by evaluating both the susceptibility of these ESBL producer strains to beta-lactam antibiotics and the pl values of the produced ESBL's. pl values of the ESBL were determined by using the Isoelectric Focusing (IEF) method. In 92% of the species investigated, a band of 7.6 pl was found. This band may belong to SHV-1 or SHV-2. The second frequent band found had a pi of 5.4 in 56% of the strains. This band was in accordance with the TEM-1. The third frequent band with a pl of 7.0 had a rate of 24% of the species and was in accordance with SHV-3. SHV derivated enzymes and TEM-1 enyzme were predominant among the beta-lactamases. As a result, to evaluate pi values of beta-lactamases together with antibiotic susceptibility may be useful in studies on observation of antibiotic resistance changes, determining antibiotic resistance mechanisms, formation of an antibiotic usage policy, and epidemiological studies.
