Özet
Akciğer tüberkülozu tanısında balgam örneklerinin yayma ve kültürü, aside dirençli basil (ARB) saptanmasında en önemli tanı yöntemleridir. Bununla birlikte, balgam çıkarmada problemi olan veya balgam örneklerinde ARB saptanamayan hastalarda, kesin tanı problem olmaktadır. Bu çalışmada klinik ve radyolojik olarak akciğer tüberkülozu düşünülen, 12'sinin (%31.5) balgamda ARB'si pozitif (10'u yayma ve kültür, ikisi ise sadece kültür pozitif) ve 26'sının ise negatif olmak üzere toplam 38 olgudan prospektif olarak fiberoptik bronkoskopi ile bronş lavajı ve bronkoalveoler lavaj alındı. Fiberoptik bronkoskopi ile alınan 38 örneğin 20'sinde (%52.6) ARB yayma ve kültür pozitif olarak saptandı. Abstract
Smear and culture of sputum samples in the determination of acid-fast bacilli (AFB) are still the most important diagnostic methods in the diagnosis of pulmonary tuberculosis. However, definitive diagnosis is a problem in patients who have difficulty in producing sputum or whose sputum samples are negative for AFB. In this study, bronchial washing and bronchoalveolar lavage samples were taken with fiberoptic bronchoscopy prospectively from a total of 38 patients, 12 (31.5%) of them AFB positive (10 of them smear and culture, two of them only culture positive) and 26 of them negative in sputum samples, suspected as to have pulmonary tuberculosis by clinical and radiologic findings. Twenty (52.6%) of the 38 patients' specimens obtained by fiberoptic bronchoscopy were positive for AFB in smear and culture. The diagnostic ratio of AFB was 57.8% when AFB positivity of smear and culture of fiberoptic bronchoscopy specimens were added to the pre-bronchoscopic sputum specimens and this increase was significant (p
