Candida türlerinin flukonazole duyarlılıklarının saptanmasında iki farklı mikrodilüsyon yönteminin karşılaştırılması

Özet
Antifungal ilaçların test edilmesi, dirençli suşlarm artmakta oluşu nedeniyle önem kazanmaktadır. Bu çalışmada, National Committee for Clinical Laboratory Standards (NCCLS) M27 A önerilerine göre RPMI 1640 besiyerinin kullanıldığı mikrodilüsyon yöntemi ile İngiliz referans yöntemi olarak bilinen ve Yeast Nitrojen Bazlı Buyyon (YNBB) kullanılan mikrodilüsyon yöntemi 40 Candida türünün flukonazole duyarlılıklarının belirlenmesinde uygulanmıştır. Candida parapsilosis ATCC 22019 kontrol susu olarak kullanılmıştır. Doksanaltı kuyucuktu mikroplakların 24 saatlik inkübasyonları tercih edilmiştir. Kırk örneğin dokuz tanesi (%22.5) NCCLS yöntemi ile flukonazole dirençli bulunurken, İngiliz referans yöntemi ile 40 örneğin altı tanesi (%15) dirençli bulunmuştur, iki yöntem arasında minimal inhibitor konsantrasyon (MİK) sonuçları açısından toplam uyum %92.5 olarak hesaplanmıştır. Çalışma sonucunda, YNBB'un duyarlılık ölçümünde en az RPM11640 besiyeri kadar kullanışlı olduğu görülmüştür.
Abstract
Susceptibility testing of antifungai agents is gaining importance because of the increasing number of resistant strains. In the present study, two broth microdilution methods were compared: The method of National Committee for Clinical Laboratory Standards (NCCLS) using RPMI 1640 M27A and United Kingdom (UK) reference method using Yeast Nitrogen Base Broth (YNBB). Both methods were used to determine the susceptibility of 40 Candida strains to fluconazole. Candida parapsilosis ATCC 22019 was used as a control strain. After 24-h incubation of 96-well microtiter plates, nine (22.5 %) of 40 isolates were resistant to fluconazole by NCCLS and six of 40 isolates (15%) were fluconazole-resistant by UK reference method. When the Minimal Inhibitor Concentration (MIC) results of the two groups were compared, the overall correlation between them was found to be 92.5 %. In conclusion, it was recommended that the YNBB medium was as convenient for use as RPMI medium for susceptibility testing of Candida spp.
