Antimicrobial resistance among clinical isolates of Salmonella spp. in Kayseri

Özet
Bu çalışmada; bazı antibiyotiklerin klinik Salmonella kökenlerine karşı etkilerinin araştırılması amaçlanmıştır. Dışkı (n=63), kan (n=12), idrar (n=1), kemik iliği (n=1) ve intra-abdominal apse (n=1) örneklerinden izole edilen toplam 78 Salmonella kökeninin ampisilin, kloramfenikol, trimetoprim-sulfametoksazol (TMP/SMX), seftriakson, siprofloksasin ve nalidiksik aside duyarlılık durumları araştırılmıştır. Duyarlılık testleri NCCLS standartlarına uygun olarak disk difüzyon yöntemi ile yapılmıştır. Suşların %60'ı S. typhimurium, %21'i S. enteritidis, % 15'i S. tvphi olarak belirlenmiştir. Üç (%4) suşun serotipi belirlenememiştir. Seftriakson, TMP/SMX ve siprofloksasine dirençli suş bulunmazken, suşların ampisilin, kloramfenikol ve nalidiksik aside direnç oranları sırasıyla %63, %58, %8 olarak bulunmuştur. Nalidiksik aside direnç sadece S. enteritidis suşlarında görülmüştür. Yöredeki Salmonella infeksiyonlarının ampirik tedavisinde ampisilin veya kloramfenikolün yerine TMP/SMX, üçüncü kuşak sefalosporinler veya kinolonlar ilk tercih olmalıdır. Ayrıca nalidiksik aside dirençli Salmonella kökenlerinde kesinlikle siprofloksasinin MİK'u belirlenmelidir.
Abstract
The aim of this study was to evaluate the in vitro activity of some antibiotics against clinical isolates of Salmonella spp. Seventy-eight strains of Salmonella spp. isolated from stool (n=63), blood (n=12), urine (n=1), bone-marrow (n=1), and intraabdominal abscess (n=1) in the period between January 1 and November 1, 2000, were tested for the susceptibility to ampicillin, chloramphenicol, trimethoprim-sufhamethoxasole (TMP/SMX), ceftriaxone, ciprofloxacin, and nalidixic acid. Susceptibility tests were performed using disk diffusion method according to the NCCLS guidelines. Salmonella typhimurium (60%) was the predominant species, followed by S. enteritidis (21%), and S. typhi (15%). Serotypes of three (4%) strains could not be determined. The resistance to ampicillin, chloramphenicol and nalidixic acid were detected in 63%, 58%, and 8% of the Salmonella isolates, respectively. All strains were susceptible to ceftriaxone, TMP/SMX, and ciprofloxacin. Nalidixic acid resistance was observed only in strains of S. enteritidis. Ampicillin and chloramphenicol cannot be considered useful for the empirical treatment of Salmonella infections. Cotrimoxazole, third generation cephalosporins and quinolones may be more prudent for the empirical treatment of suspected Salmonella infections in the region. Nevertheless, MIC for ciprofloxacin should be determined in case of nalidixic acid resistance.
