Özet

Bu çalışmada; akut bakteriyel, tüberküloz ve aseptik meninjitli olgularda prognoz ve prognoza etki eden faktörlerin araştırılması amaçlanarak, 28'i (%45.9) akut bakteri meninjiti, 13'ü (%21.3) tüberküloz meninjit ve 20'si (%32.7) aseptik meninjit olmak üzere toplam 61 meninjit olgusunda prognoza etki eden faktörler araştırılmıştır. Tüberküloz meninjitin %46'lık ölüm veya sekel oranı ile en kötü prognozlu meninjit olduğu, bunu %32'lik oranla akut bakteri meninjitinin izlediği, aseptik meninjitlerin ise %0'lık ölüm veya sekel oranı ile en iyi prognoza sahip olduğu saptanmıştır. Prognostik faktörler incelendiğinde, tüberküloz meninjit ve akut bakteri meninjitinde ileri yaşın, bilincin kapalı olmasının, tanı ve tedavide gecikmenin, konvülzyon görülmesinin, altta yatan ek bir hastalık bulunmasının ve beyin-omurilik sıvısında protein yüksekliğinin kötü prognoz işaretleri olduğu belirlenmiştir. Ayrıca akut bakteri meninjitinde beyin-omurilik sıvısında glukoz düzeyi düşüklüğü ve etkenin Streptococcus pneumoniae olması negatif prognozla ilişkili bulunmuştur. Sonuç olarak; tüberküloz meninjitin, ölüm veya sekel yönünden en kötü prognozlu meninjit olduğu, aseptik meninjitlerin ise en iyi prognoza sahip olduğu saptanmıştır

Abstract

In this study, investigation of the prognosis and determination of the prognostic factors in patients with acute bacterial meningitis, tuberculosis meningitis and aseptic meningitis were aimed. Twenty-eight (45.9%) patients with acute bacterial meningitis, 13 (21.3%) with tuberculosis meningitis and 20 (32.7%) diagnosed as aseptic meningitis, 61 cases in total, were evaluated for prognostic factors. It was found that tuberculosis meningitis had the highest mortality or sequela rate with 46%. Acute bacterial meningitis followed this with 32% and having a 0% mortality or sequela, aseptic meningitis showed the best prognosis. In the evaluation of prognostic factors; both in acute bacterial meningitis and tuberculosis meningitis high age, confused conscious, delay in diagnosis and treatment, convulsion, underlying chronic disease and high protein levels of cerebrospinal fluid (CSF) were found to be signs of negative prognosis. Nevertheless, only for acute bacterial meningitis, a low CSF glucose level and Streptococcus pneumoniae as the causative agent were also found to be related to negative prognosis. As to morbidity or mortality rates, it was seen that tuberculosis meningitis had the worse prognosis among patients with meningitis whereas aseptic meningitis showed the best prognosis.
