Depresif semptomlarla seyreden ve tedavi intoleransı gözlenen bir nörobruselloz olgusu

Özet

Bu çalışmada, ender görülen bir nörobruselloz olgusu sunuldu. Ateş, eklem ağrıları ve depresif semptomları olan 67 yaşındaki kadın hastaya doksisiklin + streptomisin ve venlafaksin (antidepresif) tedavisi uygulandı. Ateş ve eklem problemlerinin düzelmesine karşın depresif semptomlar psikoza kadar artmaya devam etti. Bu nedenle nörobruselloz kuşkusu ile hastaya lomber ponksiyon yapıldı. Beyin-omurilik sıvısında pleostoz, glukoz konsantrasyonunda azalma, protein düzeyinde artma, standart aglütinasyon testinde Brucella antikorları 1/160 titrede pozitif bulundu. Doksisiklin tedavisine intolerans geliştiğinden tedavide seftriakson + rifampisin + trimetoprim/ sulfametoksazol kombinasyonu kullanıldı. Altı haftalık tedavi sonrasında hastanın yakınmaları düzeldi ve 18 aylık izlemde relaps gözlenmedi.

Abstract

In this paper, a rarely seen case of neurobrucellosis is reported. The patient, 67-year old female, with fever, joint pain and depressive problems was treated with doxycyclin + streptomycin + venlafaksin (antidepressive). Fever and joint problems regressed, but depressive symptoms continued to increase till psychosis. Upon suspecting of neurobrucellosis, lumbar punction was carried out. In cerebrospinal fluid; pleocyteosis, decreased glucose concentration and high level of protein were found. Brucella standard agglutination test with serum was positive at 1:160 titer. Seftriaxone + rifampicin + trimethoprim / sulfametoxasole combination was used because of intolerance to doxycycline. The complaints of the patient vanished completely after six weeks of treatment, and recurrence was not seen at 18-month follow up.
