Dünyada penisilin ve diğer antimikrobial ajanlara karşı dirençli Streptococcus pneumoniae suşları tarafından oluşturulan infeksiyonların sıklığı son yirmi yıl süresince önemli ölçüde artmıştır. Bu çalışmanın amacı, bakteri meninjiti etkeni olan S. pneumoniae suşlarının penisilin, eritromisin, klaritromisin, ofloksasin, sefaklor, seftriakson, sefotaksim ve trimetoprim-sulfametoksazole (TMP/SMX)'e karşı in vitro duyarlılığını saptamaktır. Yazarların çalıştığı klinikte 1997-2000 yılları arasında akut bakteriyel meninjit tanısıyla izlenen hastaların beyin-omurilik sıvısından izole edilen 21 S. pneumoniae suşu çalışmaya alındı. Penisilin direnci oksasilin disk difüzyon tarama testi ile saptandı. Yirmibir suşun tamamı penisiline duyarlı bulundu. Disk difüzyon testi ile ofloksasin, sefotaksim, seftriakson ve sefaklore karşı direnç saptanmadı. Eritromisin ve klaritromisine %14.3, trimetoprim-sulfametoksazole %80.9 oranında direnç saptandı. Penisilin direncindeki artış nedeniyle uygun tedavi için in vitro duyarlılık testlerinin yapılması gereklidir.

Abstract

The worldwide incidence of infections caused by Streptococcus pneumoniae strains resistant to penicillin and other antimicrobial agents has increased significantly during the last two decades. The aim of this study was to determine the in vitro susceptibility of S. pneumoniae strains to penicillin, erythromycin, clarythromycin, cefotaxime, ceftriaxone, cefaclor, ofloxacin and trimethoprim-sulphamethoxazole (TMP/SMX). Twenty-one S. pneumoniae strains isolated from cerebrospinal fluid specimens of patients with bacterial meningitis between 1997 and 2000 were included in the study. The penicillin resistance was determined by using oxacillin disk diffusion screening test. The tested strains revealed no resistance to penicillin. In disk diffusion test all strains were found to be sensitive to ofloxacin, cefotaxime, ceftriaxone and cefaclor. Resistance to erythromycin, clarythromycin and TMP/SMX were 14.3 % and 80.9 %, respectively. These result indicate the necessity to perform in vitro susceptibility testing of S. pneumoniae strains against several antimicrobial agents in order to guide the antimicrobial therapy.
