Özet

İnsan sitomegalovirus (HCMV) intrauterin viral infeksiyonların en yaygın nedenidir. Akut primer CMV infeksiyonu ile reinfeksiyon/reaktivasyon arasında ayrım yapmak için varolan serolojik yöntemler bazı durumlarda yetersiz kalmaktadır. Son zamanlarda geliştirilen ve infeksiyonun zamanını belirlemede yardımcı olan CMV-lgG avidite testleri, tek bir serum örneği ile primer infeksiyon ile reaktivasyon/reinfeksiyon ayrımında fikir verebilmektedir. Bu testin özellikle gebelerde ve immünsuprese hastalarda özel bir değeri vardır. Bu çalışmada, genelde reinfeksiyon varlığını gösteren yüksek CMV- IgG avidite indeksi ve primer infeksiyonu gösteren düşük CMV- IgG avidite indeksi serolojik olarak CMV ile infekte olduğu saptanan gebe kadınlara ait 93 serum örneğinde, CMV- IgG avidite testi uygulanarak değerlendirildi. CMV-lgM ve IgG'si birlikte pozitif olan (n=41) serumların, %82.5'unda (n=34) düşük CMV-lgG avidite, %17.5'unda (n=7) ise yüksek CMV-lgG avidite indeksi bulundu. CMV-lgM (-), IgG (+) olan (n=52) serumların tümünde CMV IgG avidite testi yüksek olarak saptandı. CMV-lgG avidite testlerinin, CMV'un neden olduğu yeni primer bir infeksiyonun tanımlanmasında çok yararlı olduğu sonucuna varıldı.

Abstract

Human cytomegalovirus (HCMV) is the most common cause of viral intrauterine infection. Present serological methods sometimes differentiate poorly acute primary CMV infection from reinfection/reactivation. Recently, CMV-lgG avidity test can provide confirmatory evidence of an acute infection and distinguish reactivation/reinfections from primary infections with a single serum specimen. This is of particular value in pregnancy and immunosuppressed patients. In this study, a simple test for discriminating between high CMV-lgG avidity index which is usually present in reinfection/reactivation and low CMV-lgG avidity index typical of primary infection was evaluated. Ninety-three pregnant women's serum samples who were diagnosed as CMV infection serologically were examined for the avidity of CMV-lgG. In CMV IgM (+) and CMV IgG (+) serum samples (n=41), CMV IgG avidity index was found low (n=34) and high (n=7), %82.5 and %17.5, respectively. In all CMV IgM negative, IgG positive serum samples (n=52) avidity index was high. It is concluded that CMV-lgG avidity test is very useful in the diagnosis of a recent primary infection caused by cytomegalovirus.
