Retrospective study of 73 cases of brucellosis in the eastern region of Turkey

Özet
Van Devlet Hastanesi'nde iki yıllık sürede tedavi edilen 73 bruselloz olgusu klinik bulguları ve sağaltım protokollerini değerlendirmek amacıyla retrospektif olarak incelenmiştir. Bruselloz tanısı Wright testi titresinin 1/160 ve üstü olması ve uygun klinik bulguları ile konulmuştur. Hastaların semptomları; artralji, terleme, ateş, mide ağrısı ve iştahsızlık şeklindeydi. Lokalize komplikasyonlar; artrit ve eklem efüzyonu, idrar yolu infeksiyonu, sakroileitis/diskitis, splenomegali, hepatomegali ve orşit idi. Dört farklı tedavi protokolü uygulandı: A Protokolü: Streptomisin 21 gün İM (1 g/gün) + oral rifampin 45 gün (600 mg/gün) 34 hastaya verildi. B protokolü: Streptomisin 21 gün İM (1 g/gün) + oral doksisiklin 45 gün (200 mg/gün) 31 hastaya verildi. C Protokolü: Seftriakson 10 gün İV (2 g/gün) + oral doksisiklin 45 gün + oral trimetoprim-sulfametoksazol 45 gün (1600 mg/gün) dört hastaya verildi. D Protokolü: Streptomisin 14 gün (1g/gün) + oral doksisiklin 45 gün + oral trimetoprim-sulfametoksazol 45 gün dört hastaya verildi. Ateşin ve diğer nonspesifik bulguların kaybolması A Protokolü verilen grupta (ortalama 3.2 gün ) B Protokolü verilen gruba göre daha hızlıydı (ortalama 5.1 gün ). A Protokolü verilen grupta iki relaps olurken B Grubu'nda relaps izlenmedi. Her iki gruptan birer olgu tedaviye yanıt vermedi. İyileştirme oranları A Protokol grubu için %91.2 ve B Protokol grubu için %96.8 olarak bulundu. Sonuç olarak, B Protokolü A Protokolünden daha etkili bulundu. C ve D Protokolleri yalnızca kronik bruselloz tanısı konulan ve lokalize osteo-artiküler komplikasyonları olan olgularda kullanıldı. Bu gruplardaki toplam sekiz hasta tam olarak iyileşti ve relaps görülmedi. Bu şekildeki olgularda üçlü kombinasyon tedavisisnin verilmesi bir alternatif olabilirse de daha kapsamlı çalışmalara gereksinim vardır.
Abstract
Retrospectively 73 patients with brucellosis were evaluated to determine the clinical picture and outcome of different therapy regimens. The diagnosis of brucellosis was based on consistent clinical findings and a Wright's serum agglutination titer of 1/160 or above. The symptoms of patient were arthralgia, sweating, fewer, stomachache and lack of appetite. Localised complications were arthritis and joint effusion, urinary infection, sacroileitis/discitis, splenomegaly, hepatomegaly and orchitis. For therapy four different drug regimens were used: Regimen A: IM streptomycin 21 days (1 g/day) + oral rifampin 45 days (600 mg/day) used in 34 patients. Regimen B: IM streptomycin 21 days + oral doxycycline 45 days (200 mg/day) used in 31 patients. Regimen C: IV ceftriaxone 10 days (2 g/day) + oral doxycycline 45 days + oral trimethoprim-sulfamethoxazole 45 days (1600 mg/day) used in four patients. Regimen D: IM streptomycin 14 days (1 g/day) + oral doxycycline 45 days + oral trimethoprim- sulfamethoxazole 45 days used in four patients. Disappearance of fewer and other non specific symptoms in Regimen A (mean 3.2 days) was faster than in Regimen B (mean, 5.1 days). In Regimen A two patients relapsed although no relapse occurred in Regimen B group. One patient in each group gave no response to therapy. The cure rates were 91.2% for Regimen A and 96.8 % for Regimen B. As a result, Regimen B was found more effective than Regimen A. Regimens C and D were used only patients with chronic brucellosis and localized osteoarticular complications. In this group, totally eight patients fully recovered and no relapse occurred. The use of combination therapy in this group of patients may be the alternative therapy, but some widely studies on the subject have to be done.
