Transplant alıcılarında görülen infeksiyonların en önemli etkeni olan Sitomegalovirus (CMV), konjenital infeksiyonlara da neden olmaktadır. Sitomegalovirus hastalığının tanısında kanda pp65 antijeninin veya viral genomun gösterilmesi önemlidir. Bu çalışmada, CMV infeksiyonunun tanısında CMV antijenemi ve CMV-DNA testlerinin karşılaştırılması amaçlandı. Sitomegalovirus hastalığı öntanısı ile gönderilen 24 hastaya ait 49 kan örneği çalışmaya alındı. Hastaların 12'si yenidoğan, diğer 12'si ise kemik iliği ve renal transplantasyon hastası idi. Transplantasyon ünitelerinden gelen örnekler transplantasyon sonrası bir-üç ay dönemine aitti. Sitomegalovirus antijenemi testinde lökositlerde pp65 varlığı indirekt floresans yöntemi ile (CINAkit, Argene-Biosoft, Fransa), CMV-DNA ise hibrid yakalama yöntemi (Digene, ABD) ile araştırıldı. Örneklerin 30 (% 61)'unda CMV antijeni ve CMV-DNA negatif; beş örnekte ise her ikisi pozitif bulundu. Antijenemisi pozitif olan dokuz örnekte ise CMV-DNA testi negatifti. Beş örnekte CMV antijeni saptanamazken CMV-DNA pozitif bulundu. Her iki test ile elde edilen bulgularının istatistiksel analizinde (%2 testi), her iki test arasında fark bulunmadı. Sonuç olarak, CMV hastalığının laboratuvar tanısında ve takibinde CMV antijenemi ve CMV-DNA testlerinin birlikte uygulanmasının önemi belirlendi.

Abstract

Cytomegalovirus (CMV) is a major cause of severe infection in transplant recipients and also causes congenital infection iij newborns. Detection of CMV pp65 antigen and CMV-DNA is important for the diagnosis of CMV disease. The aim of this study was to compare CMV antigenemia and CMV-DNA assays used in the diagnosis of CMV infection. Totally of 49 blood samples from 24 patients suspected of CMV disease were included in the study. Twelve of the patients were newborns and 12 were bone marrow and renal transplant recipients. The blood samples of transplant recipients were collected in months 1-3 following transplantation. Cytomegalovirus pp65 antigen was detected by indirect immunofluorescence technique (CINAkit, Argene-Biosoft, France), and CMV-DNA by hybrid capture assay (Digene, USA). Cytomegalovirus antigen and CMV-DNA were negative in 30 (61%) of the samples. Both assays were found positive in five. Nine samples were antigenemia positive, but CMV-DNA negative. Five samples were CMV-DNA positive, but antigenemia negative. No significant difference was found between the two methods by x2 test. It is concluded that CMV antigenemia and CMV-DNA assays should be performed together for the diagnosis of CMV infection.
