Kraniyal sinir tutulumu ile seyreden üç nörobruselloz olgusu

Özet

Bruselloz, Türkiye'nin de içinde bulunduğu Akdeniz ve Orta-doğu coğrafyasında endemik olarak görülen, tüm dünyada yaygın bir zoonozdur. Santral sinir sistemi tutulumu, sistemik brusellozlu olguların % 5'inden azında görülür. Kraniyal sinir tutulumu ise oldukça nadirdir. Burada, vestibulo-kohlear sinirin tutulumu ile bilateral sensori-nöral tipte işitme kaybı, Nervus facialis tutulumu ile sağ periferik fasiyal paralizi ve N. abducens tutulumu ile şaşılık gelişen üç nörobruselloz olgusu sunuldu. Antibakteriyel tedavi ile fasiyal paralizi ve şaşılığın tamamen düzeldiği, işitme kaybında bir iyileşmenin olmadığı, aksine, progresyon göstererek tam duruma geldiği izlendi.

Abstract

Brucellosis is a worldwide zoonosis and endemic in the Mediterranean and Middle-East regions. Central nerve system involvement is encountered in less than 5 % of systemic brucellosis cases. In this article, three cases of neurobrucellosis are presented. One of the cases had 8th cranial nerve involvement and hearing loss, the other case had 7th cranial nerve involvement and facial paralysis. Facial paralysis improved with antibiotic treatment, but hearing loss did not.
