Pürülan meninjitlerin erken tanısında lateks aglütinasyon testinin değerlendirilmesi

Özet

Bu çalışmada, akut pürülan meninjitlerde etkenin belirlenmesinde kültür ve Gram boyama yöntemleriyle lateks aglütinasyonu yönteminin karşılaştırılması amaçlanmıştır. Çalışmaya 29'u erkek, 20'si kadın olmak üzere klinik olarak pürülan meninjit tanısı konulan 49 olgunun beyin-omurilik sıvısı (BOS) örnekleri alınmıştır. Olguların yaş aralığı 5 gün-75 yaş arasında değişmektedir. Olguların 22 (% 44)'sinde lateks aglütinasyonu (Slidex-Meningite-Kit 5 bio Merieux), 17 (% 34)'sinde kültür yöntemleriyle etken belirlenmiş, 10 (% 20) örnekte Gram boyama ile bakteri görülmüştür. Kültürde Streptococcus pneumoniae üreyen dokuz olgunun sekizinde (% 88), Neisseria meningitidis üreyen altı olgunun beşinde (% 83), Haemophilus influenzae ve Escherichia coli üreyen birer olguda lateks aglütinasyon testi pozitif saptanmıştır. Lateks aglütinasyonu duyarlılığı S. pneumoniae için % 88, N. meningitidis için % 83 olarak belirlenmiştir. Pürülan meninjitlerin tanısında lateks aglütinasyonu hızlı, kullanımı kolay ve duyarlı bir test olması nedeniyle, kültür ve Gram boyama ile.birlikte rutin olarak kullanılabilir. Bu durum, meninjit gibi erken tanı ve tedavinin yaşamsal öneme sahip olduğu bir hastalıkta, sekel ve mortaliteyi azaltmaya katkıda bulunabilir.

Abstract

The purpose of the present study was to compare the Gram stain and culture results of cerebrospinal fluid (CSF) with latex agglutination technique to determine the etiological agents of acute purulent meningitis. Cerebrospinal fluid samples of 49 cases diagnosed clinically as purulent meningitis (29 men, 20 women) were examined. The age range of the cases were 5 days-75 years. The etiological agents were determined with latex agglutination method (Slide-Meningite-Kit 5, bio Merieux) in 22 (44 %) of the cases, with culture methods in 17 (34 %), and bacteria were seen with Gram stain of spun sediment in 10 (20 %) of the cases. Latex agglutination test was positive in eight (88 %) of nine Streptococcus pneumoniae, five (83 %) of six Neisseria meningitidis and in one Haemophilus influenzae type b and one Escherichia coli case(s). The sensitivity of the latex agglutination test was 88 % for S. pneumonias, 83 % for N. meningitidis. In conclusion; as the latex agglutination method is a fast, easy and sensitive test for the diagnosis of purulent meningitis, it can be used routinely with Gram stain and culture of CSF. The use of these tecniques could help to reduce the mortality and sequelae in a disease like meningitis the early diagnosis and treatment of which are of utmost importance.
