Özet
Entamoeba histolytica asemptomatik taşıyıcılıktan, ciddi fulminan amibik kolite ve bağırsak dışı amebiyaza kadar farklı klinik tablolara neden olabilen önemli patojenlerden biridir. Bu çalışmanın amacı, Sivas yöresinde amebiyaz epidemiyolojisini saptamaktır. Ocak-Aralık 2001 döneminde Sivas Kızılay Tıp Merkezi Laboratuvarı'na gönderilen dışkı örneklerinin makroskobik ve mikroskobik inceleme sonuçları retrospektif olarak değerlendirilmiştir. Bu dönemde 1216'sı erişkin ve 1031'i ise çocuk olmak üzere 2247 olguda dışkı örneği incelenmiştir. İshalli olguların %20'sinde amipli dizanteri, tüm örneklerin de %10.9'unda asemptomatik amip taşıyıcılığı saptanmıştır. Dışkı incelemesinde makroskopik kan ve mukus, mikroskopik lökosit ve eritrosit görülme sıklıkları amipli dizanteri ile yüksek bir uyum göstermektedir. Amipli dizanteri görülme sıklığı çevresel etkenlerden etkilenirken, asemptomatik taşıyıcılığı daha çok yapısal nedenler belirliyor gibi görünmektedir. Bu haliyle amebiyaz Sivas Bölgesi için önemli bir sorundur.
Abstract
Entamoeba histolytica is an important pathogen which causes a wide spectrum of illnesses ranging from asymptomatic carrier state to serious fulminant amebic colitis or extraintestinal amebiasis. The purpose of this study was to determine the epidemiology of amebiasis in the Sivas Region. The results of stool examinations in Sivas Kızılay Medical Center Laboratory between January and December 2001 were retrospectively evaluated. During this period, specimens from 1216 adults and 1031 children, totally 2247 specimens, were examined. Amebic dysentery was detected in 20% of diarrheal stools while 10.9% asymptomatic carrier state was detected in all of the samples. Macroscopic blood and mucus and microscopic white or red blood cell detection highly correlated with amebic dysentery. The frequency of amebic dysentery was affected by enviromental conditions while the carrier state seemed to be determined mostly by housing conditions. Amebiasis is an important problem in the region.
