Bir rehabilitasyon merkezinde gelişen hastane infeksiyonlarının değerlendirilmesi

Özet

Bu çalışmanın amacı, hasta populasyonunun çoğunluğunu omurilik yaralanması olan hastaların oluşturduğu Türk Silahlı Kuvvetleri Rehabilitasyon ve Bakım Merkezi (TSKRBM)'nde 1 Ocak 2002-31 Aralık 2003 tarihleri arasında yatarak tedavi gören hastalarda gelişen nozokomiyal infeksiyonları değerlendirmek idi. Merkez'de 2002 yılında 3746 hastadan 294'ünde gelişen toplam 380 hastane infeksiyonundan 458 mikro-organizma izole edilirken 2003 yılında 3966 hastanın 241'inde 283 hastane infeksiyonu görüldü ve 326 mikro-organizma izole edildi. Hastane infeksiyon hızı 2002 ve 2003 yılında sırasıyla %10.14 ve %7.14 olarak bulundu. Kliniklere göre hastane infeksiyonlu hasta yatış oranları değerlendirildiğinde, akut bakım kliniğinin her iki yıl da en yüksek oranlara sahip olduğu (sırasıyla %37.96 ve %35.98) görüldü. Her iki yılda da en sık görülen infeksiyon tipi asemptomatik bakteriyüri (sırasıyla %55.26 ve %57.59) ve en sık izole edilen nozokomiyal etken Escherichia co//(sırasıyla %33.41 ve %40.49) oldu. Enterobacteriaceae için piperasilin/tazobaktam ve imipenemin en etkili, ampisilin ve trimetoprim/sulfametaksazolun ise en az etkili ilaçlar olduğu görüldü. Nonfermentatif Gram-negatif basiller için ise yine piperasilin/tazobaktam ve imipenem en etkili, trimetoprim/sulfametaksazol ve aztreonam en az etkili olarak bulundu. Ayrıca hastane infeksiyon etkeni olarak izole edilen metisiline dirençli Staphylococcus aureus kökenlerinin tüm nozokomiyal etkenler arasında 2002 ve 2003 yılında sırasıyla %3.74 ve %4.60 oranında bulunduğu saptandı. Sonuç olarak, hastane infeksiyonlarının önlenmesinde kontrollü antibiyotik uygulaması, aktif sürveyans, izolasyon uygulamaları ve eğitime önem verilmelidir.

Abstract

The purpose of this study was to evaluate the nosocomial infections developing in patients most of whom had spinal cord injury at the Turkish Army Force Rehabilitation and Care Center between January 1, 2002, and December 31, 2003.. Totally 458 microorganisms were isolated from 380 hospital infections in 294 of 3746 patients in 2002 while totally 326 microorganisms were isolated in 283 hospital infections observed in 241 of 3966 patients in 2003. The rate of hospital infections in 2002 and 2003,were found as 10.14% and 7.14%, respectively. When the ratio of hospitalized patients with nosocomial infection to the clinics was evaluated, it was observed that acute çare clinic had the highest ratios in both years (37.96% and 35.98%, respectively). In both years, the most frequent type of nosocomial infection was found to be asymptomatic bacteriuria (55.26% and 57.59%, respectively) and the most'frequently isolated nosocomial microorganism Escherichia coli (33.41 % and 40.49%, respectively). Against Enterobacteriaceae, piperacillin/tazobactam and imipenem were found as the most effective, ampicillin and trimetopim/sulfametpkazol as the least effective agents. While piperacillin/tazobactam and imipenem were found to be the most effective agents against Gram-negative nonfermentative bacilli, trimethoprim/sulfamethoxazole and aztreonam were found to be the least effective agents. In addition, the isolation rates of methicillin-resistant Staphylococcus aureus as cause of nosocomial infection were 3.74% and 4.60% in 2002 and 2003, respectively. It is concluded that in the prevention of nosocomial infections controlled use of antibiotics, active surveillance, isolation and training of the personal are important issues.
