Özet

Koroner arter hastalarında preoperatif risk faktörlerinin iyi değerlendirilmesi, erken operatif prognoz açısından oldukça önemlidir. Merkezimizde Haziran 1993 - Ocak 1995 tarihleri arasında öpere edilen, 46'sı kadın, 204'ü erkek, toplam 250 koroner bypass olgusunda operatif mortalite %6 (15/250) olarak belirlenmiştir. Hastaların 51 'inde (%20.4) diabetes mellitus, kronik böbrek yetmezliği, kronik obstrüktif akciğer hastalığı veya serebrovasküler bozukluk gibi risk fak¬törü olarak kabul edilen en az bir hastalık mevcuttu. Olguların 26'sı (%10.4) acil koşullarda öpe¬re edilmiş, 2 hastaya (%0.8), ikinci koroner bypass operasyonları uygulanmışdı. Yüksek riskli hasta grubunda bildirilen sonuçlar, yeterli miyokardiyal koruma, dikkatli monitorizasyon ve uy¬gun anestezi teknikleriyle risk faktörlerinin etkisinin en az düzeye indirilebileceğini ortaya koy¬maktadır.

Abstract

The Early Postoperative Outcome and the Effects of Risk Factors in Coronary Artery Surgery Evaluation ofpreoperative risk factors related to early operative outcome might permit the ma¬nagement of the timing of the coronary bypass operation. The operative mortality of 250patients (46 female, 204 male), who underwent coronary bypass surgery during the period from June 1993 until January 1995, was found to be 6% (15/250). Fifth one patients 20% had aPMXsioke, of the concomittant disorders, including diabetes mellitus, chronic renal failure, chronic obstruc¬tive pulmonary disease or cerebrovascular events. Twenth six patients (10.4%) underwent emer¬gency coronary artery bypass grafting, while 2 of them had their second operations. Reported re¬sults about coronary bypass in high risk patients suggest that the effects of the risk factors can be managed by sufficient myocardialpreservation, careful monitorization and suitable anesthesia.
