Hemşirelerin özelleştirmelere ilişkin algı ölçeğinin geliştirilmesi

Özet

Amaç: Bu araştırma, hemşirelerin sağlık hizmetlerinde özelleştirme uygulamalarına ilişkin algılarını belirlemek üzere bir ölçek geliştirmek amacıyla yürütülmüştür. Yöntem: Metodolojik bir çalışmadır. Meslek örgütüne üye, İstanbul’da, kamuda çalışan hemşirelerin oluşturduğu örneklem grubundan uygun şeklide doldurulup geri toplanan 460 anket değerlendirilmeye alınmıştır. Ölçeğin değerlendirilmesinde kapsam geçerliği, yapı geçerliği, iç tutarlık analizi ve algı puan ortalamaları kullanılmıştır. Bulgular: Ölçeğin kapsam geçerlik indeksi, genel özelleştirme, sağlıkta özelleştirme ve özelleştirmelerin hemşireliğe etkisi alt boyutlarında sırasıyla 0,88; 0,84 ve 0,96 olarak bulunmuştur. Ölçeğin madde toplam puan korelasyonları 0,41-0,75 arasında ve Cronbach _ güvenirlik katsayısı 0,95, alt boyutlarda ise sırasıyla 0,82, 0,89, 0,92 olarak 0,70’den yüksek ve güvenilir bulunmuştur. Yapı geçerliği başlığında doğrulayıcı faktör analizi yapılmış, üç faktörlü ölçek yapısı doğrulanmış ve faktör yükü 0,40’ın altında olan maddeler çıkarılmıştır. Uyum iyiliği istatistiklerinde, ölçeğin uyumunun iyi olduğu bulunmuştur. Ölçek toplam puan ortalaması 2,16±0,57’dir. Alt ölçeklerin ortalaması sırasıyla; genel özelleştirme, 2,20±0,66; sağlıkta özelleştirme, 2,10±0,59 ve özelleştirmelerin hemşireliğe etkisi 2,21±0,65’tir. Sonuç: Bu çalışmaya göre hemşireler özelleştirme uygulamalarını, genel özelleştirmeleri, sağlıkta özelleştirmeleri ve özelleştirmelerin hemşireliğe etkisini olumsuz algılamaktadırlar. Bu çalışmada geliştirilen ölçek geçerli ve güvenilirdir. Sağlık hizmetlerinde farklı örneklem gruplarında kullanılması önerilir.

Abstract

Objective: The present study was carried out to develop a scale for defining the perceptions of nurses of privatization practices in health services. Method: This is a methodological study. Four hundred and sixty questionnaires completed and returned by the sample group consisting of nurses who were members of the same occupational organization and worked at public hospitals in Istanbul, were analyzed. Content validity, construct validity, internal consistency and perception score means were used in the evaluation of the scale. Results: Content validity index of scale was found as 0.88, 0.84, and 0.96 in “general privatization”, “privatization in health”, and “effects of privatizations on nursing” subscales, respectively. Mean total score correlations of the scale were between 0.41-0.75; Cronbach’s _ coefficient of the scale was 0.95; Cronbach’s _ coefficients of the subscales were 0.82, 0.89, 0.92, respectively, and were over 0.70 and significant. Explanatory factor analysis was done in construct validity, three-factor scale structure was confirmed and the items with factor load less than 0.40 were removed. Goodness of fit statistics showed that the goodness of fit indexes of the scale were suitable. Mean total score of the scale was 2.16±0.57 and the means of the subscales were 2.20±0.66 in general privatization, 2.10±0.59 in privatization in health, and 2.21±0.65 in the effects of privatization on nursing. Conclusion: The present study confirmed that the nurses perceived privatization practices, general privatizations, privatizations in health, and the effect of privatizations on nursing negatively. The scale developed in this study is reliable and valid, and the scale is recommended to be used in different sample groups in health services.
