Amebiazis klinik bulgusu olarak afebril konvülsiyon

Özet
Amebiazis özellikle gelişmekte olan ülkelerde önemli gastroenterit nedenlerinden biridir. Amebiazis asemptomatik, intestinal ve ekstra intestinal amebiazis klinik tablolarıyla seyredebilir. Literatürde, amebiazis olgularındaki afebril konvülsiyonla ilgili yayın henüz yoktur. Konvülsiyon nedeni ateş, dehidratasyon ve elektrolit imbalansı dışında parazitin doku invazyonu sırasında salınan maddelere karşı olabilir.Hücre yıkımı kalsiyum,belli parazit proteinazları ve glikozidazları ile olur ve benzer olayın amebiaziste konvülsiyonun nedeni de olabileceği düşünülebilir. Metod: Bu çalışmada kliniğimizde 1.1.2000 -1.1.2002 tarihleri arasında yatarak tedavi gören.intestinal amebiazis tanısı almış 30 hasta retrospektif olarak değerlendirilmiştir. Hastalar yaş .cinsiyet yanında biyokimyasal olarak kan şekeri,BUN,kreatinin, AST,ALT,sodyum, potasyum, kalsiyum; mikrobiyolojik olarak hemoglobin, lökosit, trombosit, CRP.gaitada lökosit - eritrosit varlığına ve amip trofozoid ve kistlerinin görülmesi göre; klinik olarak ta ateş,kusma, kanlı ishal,dehidratasyon,konfüzyon ve konvülsiyon yönünden değerlendirildi. Bulgular: Yaş ortalaması 63.80±28.56 ay(10-144ay) olan.16 kız (%53.3),14 erkek (%46.7) 30 hasta çalışmaya almmıştır.Olgularm tümünde ishal mevcut iken 24 hastada ateş (%8Û),20 hastada ise kusma (%66.6)vardı. Ateş, ishal ve kusmanın birlikte bulunduğu olgu sayısı 15(%50) idi.En sık görülen klinik bulgu ateş ile birlikte kanlı ishal idi ve 24 (%80) vakada saptandı. Dehidratasyon olguların %60'ında (18 olgu) gözlenmiştir. Bu olguların 14'ünde (%46.7) hafif-orta dehidratasyon, 4'ünde ise (%13.3) ağır dehidratasyon mevcuttu.Konvülsiyon geçiren 5 olgunun 4'ünde afebril konvülsiyon (%13.3),konfüzyon ise 13 olguda(%43.3) saptandı.Gaitada lökosit 23 olguda (%76.6) pozitif iken eritrosit 25 olguda (%83.3) pozitif bulundu. Sonuç: Amebiasis.tüm dünyada hala yaygın bir problemdir. Amebiazis olgularındaki afebril konvülsiyonun nedeni gelecekteki ayrıntılı patofizyolojik çalışmalarla belirlenebilir.
Abstract
Background: Amebiasis is one of the major reason of gastroenteritis in developing countries. It can be seen with asymptomaticjntestinal or extraintestinal forms in clinic.There has not any published amebiasis case associated with afebrile convulsion in the literatür yet. The reason of convulsion can be fever, dehydration, electrolyte imbalance or the degradation of tissue invasion that is caused by parasite.The certain parasitic proteinases,glycosides can cause tissue degradation with calcium release which can be encuontered in the triggering mechanism at convulsion with the case of amebiasis. Method: In this study, 30 patients who were diagnosed,hospitalized and treated as intestinal amebiasis between January 2000 and January 2002 were evaluated retrospectively.These patients were investigated according to age,sex,blood glucose,creatinin,urea nitrogen,AST,ALT,sodium,potassium,calcium levels and blood hemoglobin.white blood count,platelets,CRP levels and also were evaluated whether there is red blood cell.white blood cell and amebic cyst or trophozoides in stool and whether there is any fever,vomiting,bloody diarrhea, dehydration,confusion and convulsion clinically. Findings: We had 30 patients,16 of them were female (53.3%) and 14 of them were male (46.7%) with the median age of 63.80+28.56 months(10-144 months).AIthough all patients had diarrhea only 24 of them (80%)had fever and 20 of them (66.6%) had vomiting in 15 (50%) cases. Most common clinical findings were fever and bloody diarrhea and were positive in 24 (80%) cases. There is dehydration in 18 (60%) cases. Mild or modest dehydration were found in 14 (46.7%) cases and severe dehydration were found in 4 (13.3%) cases. Afebrile convulsion were seen in 4 (13.3%) cases and confusion werefound in 13 (43.3%) cases. White blood cells were positive in stool of 23 (76.6%) cases and red blood cells were positive in stool of 25 (83.3%) cases. Conclusion: Amebiasis are still the common problem all over the world and the reason of afebrile convulsion in clinic can be determined with the detailed pathophysiologic studies in the future .
