Özet

Bu çalışmada 57 Nefrotik sendromlu (NS) çocuk; renal ultrasonografik (USG) bulgular ve steroid cevabı arasında muhtemel bir ilişkiyi ortaya koymak amacıyla retrospektif olarak incelendi. 57 hastanın 21'nde anormal USG bulguları saptandı; % 22.8'nde nefromegali,% 33.3'nde renal ekojenite artışı saptandı. USG'nin çocukluk çağı NS'da steroid rezistansını göstermesi açısından sensitivitesi % 69, spesifisitesi % 72, pozitif prediktif değeri % 42 ve negatif prediktif değeri % 88 olarak saptandı. Sonuç olarak; daha önceki yayınlarda da belirtildiği gibi renal ultrasonografinin NS'da steroid cevabı hakkında sensitif ipuçları vermediği kanısına varıldı.

Abstract

In this report we retrospectively evaluated 57 children with primary nephrotic syndrome to test if there has been a relationship between initial ultrasonographic evaluation and subsequent corticosteroid responsiveness. Twentyone of 57 patients (36.8 %) had abnormal sonograms; nephromegaly was present in 22.8 % and increased renal echogenity in 33.3 %. Steroid resistance was present in 22.8 % of the patients. Sensitivity of USG for diagnosing steroid resistance in childhood primary nephrotic syndrome was 69 %; specificity 72 %, positive predictive value 42 % and negative predictive value was 88 %. In conclusion initial USG findings is not a sensitive indicator for giving clues for subsequent corticosteroid responsiveness as reported before.
