Özet
İntrauterin gelişme kısıtlılığı perinatal mortalitesi yüksek, önemli bir perinatal problemdir. Etiyolojik faktörler arasında plasental nedenler önemli bir yer tutarken plasentaya sınırlı mozaisizm çok ender görüleni bir klinik durumdur. Bununla birlikte 9-11. gebelik haftaları arasında koryon villus örneklemesi yapılan hastaların yaklaşık %1-2' sinde sıklıkla trizomi şeklinde olmak üzere plasentaya sınırlı sitogenetik anormallikler saptanmıştır. Tanısı çoğunlukla koryon villus örneklemesinde kromozomal mozaisizm tanısı konulan olgularda, amniotik hücre kültürü veya fetal kan örneklemesi kültürü gibi ikinci prenatal testlerde normal diploid karyotip görülmesi ile konabilir. Bu raporda erken dönem gelişme kısıtlılığı gösteren ve plasentaya sınırlı 45,XO mozaisizm olgusu tanımlanmıştır.
Abstract
Fetal intrauterine growth restriction has a high perinatal mortality. Therefore, intrauterine growth restriction is a clinically significant prenatal problem. The etiology for intrauterine growth restriction is best thought of in three categories: fetal factors, placental factors and maternal factors. Altough placental factors are more common, Confined placental mosaicism is an extremely rare clinical entity. However, in approximately 2% of viable pregnancies studied by chorionic villus sampling at 9 to 11 weeks of gestation, the cytogenetic abnormality, most often trisomy, is confined to the placenta. The diagnosis of confined placental mosaicism is commonly made when, after a diagnosis of chromosomal mosaicism in a chorionic villus sampling sample, the second prenatal testing( amniotic fluid culture or fetal blood culture analysis ) shows a normal diploid karyotype. This report describes a case of apparent confined placental mosaicism for monosomy 45, XO in a pregnancy which produced a female infant with early intrauterine growth restriction.
