Özet

Gelişmiş tanı ve tedavi yöntemlerine rağmen çocukluk çağı menenjitleri yüksek oranda mortalite ve morbiditeye neden olarak günümüzde de önemini korumaktadır. Çalışmamızda, menenjitin klinik şekillerini, menenjitte tanı ve tedavi yaklaşımlarımızı ve sonuçlarını irdelemeyi amaçladık. Çalışmamızda, Haydarpaşa Numune Hastanesi Pediatri Kliniğinde Ocak 1999-Aralık 2001 tarihleri arasında takip edilen, klinik özellikleri, BOS bulguları ve kültür sonuçlarına göre bakteriyel, viral veya tüberküloz menenjit tanısı alan 151 olgu retrospektif olarak değerlendirildi. Hastaların yaş ortalaması 68,08 ( 46,27 ay (1 -168 ay) bulundu, 47'si (%31,1) kız ve 104'ü (%68,9) erkekti. Hastaların 63'ü (%41,7) bakteriyel menenjit, 83'ü (%55) aseptik menenjit, 5'i (%3,3) tüberküloz menenjiti tanısı aldılar. Aseptik menenjit tanısı alan hastaların 30'unda (%56) kabakulak menenjiti tespit edildi. BOS kültürlerinin 133'ünde (%89,3) üreme olmazken, 7 olguda (%4,7) S. pneumonia, 3 olguda (%2) koagülaz (-) stafilokok, 2 olguda (%1,3) N. meningitidis, 2 olguda (%1,3) acinetobacter, birer olguda ise (%0,7) L monositogenes ve S. pneumonia ve koagülaz (-) stafilokok ikili üremesi gerçekleşti. Tedavi sonunda 135 hasta (%89,4) şifa ile taburcu oldu.7 hasta (%4,6) eksitus oldu. Sonuçta, hastaların en sık başvuru yakınması ateş,kusma olup bunu başağrısı izlemiş.daha az oranda da bilinç bozuklukları ve konvülsiyon gözlenmiştir.En sık aseptik menenjit tanısı konulmuş olup bunlarında %56'sının kabakulak menejiti olduğu saptanmıştır. Hastalarımızın %89.4'ü (n:135) ortalama 12 günlük tedavi sonucu şifa ile taburcu edildi, %4,6 (n:7) olgu kaybedildi.

Abstract

Although there are some advanced methods of diagnosis and treatment of the childhood meningitis, nowadays it is an important issue due to high ratio of mortality and morbidity. In our study, we aimed to search the clinical types of meningitis and the approaches and results of the diagnosis and treatment on meningitis. We have retrospectively evaluated 151 cases that are diagnosed as bacterial, viral or tuberculosis meningitis by the results of the clinical characteristics, findings of BOS and the culture. In our study, we have followed up these 151 cases in Pediatrics Clinic of the Haydarpaşa Numune Hospital between January 1999-December 2001. The average age of our patience found to be 68,08(46,27 months (1-168 months). 47 of our patience were women, and 104 were men. Out of the 151 patience, we have diagnosed 63 (% 41,7) of them with bacterial meningitis, 83 (% 55) with, and 5 (% 3) with tuberculosis meningitis. 30 percent of those who had been diagnosed as aseptical meningitis also diagnosed with mumps meningitis. Where there was no reproductivity on 133 (% 89,3) cases of the cultures of BOS, there had been reproduction on 7-.(% 4,7) cases with S. pnemuonia, 3 (%2,2) cases with coagulase (-) staphylococcus, 2 (% 1,3) cases with N. meningitidis, 2 (% 1,3) cases with Acinetobacter, and finally each 1 ( % 0,7 ) case with L. monocytogenes, S. pneumonia, and bilateral coagulase (-) staphylococcus. Finally, most patients complained with fever, vomiting, and headache, and few had been observed with conscious breakdowns and convulsion.Aseptic meningitis was the most diagnosed one, % 56 percent of these have found to be mumps meningitis. In a 12-day average treatment period, % 89.4 (n:135) percent of our patients were checked out from our hospital in good health, while % 4,6 (n:7) percent had shown to have no symptoms at all.
