Özet
Varisella yaygın görülen bulaşıcı bir çocukluk çağı hastalığı olup, kısa bir prodromal dönemi takiben makûl, papül, vezikül, püstül ve kabuklanma ile seyreden kaşıntılı cilt lezyonları ile karakterizedir. Sağlıklı çocuklarda sistemik semptomlar çok hafiftir fakat, adultlarda ve hücresel immun cevabı bozuk olan hastalarda ciddi komplikasyonlar oluşabilir. Gebe bir kadında doğumdan bir hafta önce veya bir hafta sonra varisella enfeksiyonu görülürse, doğan bebekte sıklıkla enfeksiyon ortaya çıkar ve ciddi boyutta olabilir. İlk enfeksiyon intrauterin 'başladığı halde klinik bulgular postpartum dönemde görülür. Bu yazıda, doğumdan 8 gün önce suçiçeğine yakalanan anneden doğan ve biri postnatal 10. günde, diğeri postnatal 29. günde varisella döküntüleri başlayan preterm SGA ikiz iki olgu sunulmuştur. E.Coli sepsisi gelişen ve 17. günde kaybedilen birinci bebek neonatal varisella olarak değerlendirilmiştir. İkinci bebek ise postnatal kazanılmış varisella olarak değerlendirilmiş ve iyileşmiştir. İkinci olguda serumda 5. gün bakılan varisella IgM pozitif bulunmuştur.
Abstract
Varicella is a common, contagious childhood disease characterized by itchy, maculopapulovesicular skin lesions occurring after a short prodromal period. Systemic manifestations are not severe in healthy children but in adults and immunucompromised patients severe complications may develop. If the mother experiences varicella infection 1 week before or after labour, her child will also develop the infection. Although the infection begins in intrauterine period, clinical symptoms are encountered in postpartum period. We present term SGA twins born from a mother experiencing varicella 8 days before the labor. Varicella skin lesions were observed in first child at 10th day postnatally whereas at 29th day postnatally in the other sibling. The first sibling who died due to E.Coli sepsis at 17th day was considered as a neonatal varicella case. The second sibling is considered to have postnatal acquired varicella and is discharged from hospital. Serum varicella IgM at 5th postnatal day was found to be positive in the second child.
