Özet
Amaç : Hemodiyaliz tedavisi altında olan olgularda subklavyan ve brakiyosefalik ven stenoz veya oklüzyonlarında endovasküler tedaviyi değerlendirmek Materyal ve Metod : Hemodiyaliz tedavisi alan, santral venöz kateter girişimi uygulanmış, proksimal tıkanıklık ile uyumlu klinik bulguları nedeniyle yapılan venografik incelemelerinde stenoz veya oklüzyon saptanan 6 olgu değerlendirildi. Oklüzyon saptanan olgulara rekanalizas-yon sonrası, stenoz saptanan olgulara doğrudan balon dilatasyon uygulandı. Yeterli cevap alınamayan durum-larda stent yerleştirildi. Olgular 3, 6, 12. aylarda klinik ve koldaki hemodiyaliz fistül patensisi değerlendirilerek takip edildi. Bulgular : Altı olguda 7 lezyon saptandı. Lezyonlarm 3'ü subklavyan vende stenotik; 3'ü subklavyan vende, 1'i brakiyosefalik vende oklüziv karakterdeydi. Lezyonları 4'ü sol, 3'ü sağ tarafta lokalizeydi. izole sağ subklavyan ven oklüzyonu bulunan olgulardan birinde rekanalizas-yon sonrası balon dilatasyonuyla yeterli patensi sağlandı. Diğer üç olguda balon dilatasyon sonrası stent yerleştirildi. Stenoz saptanan 3 olgudan 2'sinde balon dila-tasyonla yeterli patensi sağlanırken bir olguda stent yer-leştirildi. Bu olguda, 12. aydaki kontrolde restenoz sap-tanması üzerine balon dilatasyon ile patensi tekrar sağlandı. Diğer 5 olguda restenoz veya reoklüzyon izlenmedi. Sonuç : Endovasküler rekanalizasyon, santral venöz stenoz ve oklüzyon suptanan hemodiyaliz hastalarında uzun dönem patensi sağlayabilen, cerrahi girişime alter-natif başarılı bir tedavi yöntemidir.
Abstract
Objectives : To analyze the effectiveness of endovascu-lar therapy in central venous stenosis or occlusions in hemodialysis patients. Material and Methods : Venographies of six patients, undergoing hemodialysis, are obtained because of cent-ral venous stenosis or occlusion. Balloon dilatation performed after recanalization in occlusions, and directly in stenosis. Stent deployment performed after insufficient balloon dilatation. Clinical symptoms and fistula patenci-es followed up on the 3rd, 6th and 12th months. Findings: Seven lesions determined. Four were occlusion and 3 were stenosis. All 3 stenotic lesions were in subclavian veins. Three of the occlusive lesions were in subclavian and 1 was in brachiocephalic vein. Four lesions were on the left and 3 were on the right. Sufficient patency achieved with balloon dilatation in one of the subclavian vein occlusions. In others, stent deployment performed. Balloon dilatations were sufficient in 2 of 3 stenotic lesions. Although in the 3rd one stent deployment needed, in which restenosis diagnosed on the 12th month and treated with balloon dilatation. All other veins were patent at controls. Conclusion : With acceptable longterm patency rates, endovascular treatment of central venous stenosis or occlusions, in hemodialysis patients, is a successful and alternative therapy.
