Özet

Çalışmamızda üç farklı bolus dozda remifentanil kullanılarak entübasyonda hemodinamik yanıttaki değişiklikler araştırıldı. ASA l-ll grubunda 60 hasta rastgele seçilerek 4 gruba ayrıldı. Atropin 0.5 mg ve diazepam 10 mg (IM) ile premedike edilen hastalara preoksijenizasyondan sonra serum fizyolojik (plasebo, Grup 1) veya remifentanil 0.5 (Grup 2), 1.0 (Grup 3), 1.25 $mu$g/kg (Grup 4) 30 saniyede uygulandı. Pentotal 5-7 mg/kg, süksinilkolin 1mg/kg verilerek entübasyon sağlandı. % 50 -50 O2- N2O ve % 0.5 izofluran ile ventilasyona devam edildi. Arteriyel basınç, kalp hızı ve Sp02 değerleri, bazal değer, remifentanil sonrası, entübasyon öncesi, sonrası ve birer ; dakika ara ile 5 kez olmak üzere toplam 9 zamanda kaydedildi. İstatistiksel değerlendirme ANOVA ve Tukey Kramer çoklu karşılaştırma testi ile yapıldı. Abstract

The effect of three bolus doses of remifentanyl on the haemodynamic response to endotracheal intubation was assessed in our study. Having patient conset and instutional approval, 60 ASA l-ll patients were included in the study. After premedication with diazepam 10 mg IM and atropin 0.5mg IM, they were divided in to four groups. After preoxygenation for 3 minutes, the patients were gives normal saline in Group 1, remifentanyl 0.5 $mu$g$kg^-1$ in Group 2,1$mu$g$kg^-1$ in Group 3, 1.25 $mu$g$kg^-1$ in Group 4 in 30 seconds. Anaesthesia was induced with thiopental 5-7 mgkg-', succinylcholine 1mg$kg^-1$ was given for neuromusculer relaxation and 1 minute later laryngoscopy and tracheal intubation was performed. Patients were ventilated with 0.5 % isoflurane and 50 % nitrous oxide in oxygen after intubation. Arterial pressures (SAP, DAP) and heart rate (HR) were recorded after preoxygenation (baseline), after injection of remifentanyl, before laryngoscopy and intubation, after tracheal intubation and 1 minute intervals for 5 minute after intubation.
