Özet
Ameioblastoma en yaygın görülen epitelyal odontojenik tümördür. Genellikle 20-40 yaşları arasında görülür. Dental lamina, enamel organ, dental folikül ve dentigeröz kistin epitelyal tabakasından kaynaklanabilir. Her iki cinsiyette eşit oranda görülür. Tespit edilen ilk semptom, ağrısız kemik ekspansiyonudur. Mandibula maksilladan dört kat daha sık tutulur. Ameioblastoma, malign tümör sınıflamasında yer almamasına rağmen, ileri derecede agresif ve infiltratif seyir gösterebilir. Tedavisi geniş serbest marjinle cerrahi eksizyondur. Hastalar tüm yaşamları boyunca radyografik olarak takip edilirler. Eksizyon yetersizse rekürens sık görülür. Fibular osteokutanöz flep, mandibula rekonstrüksi-yonunda tercih edilen cerrahi tekniklerden biridir. 25 cm'ye kadar bikortikal kemik alınabilmesi, multipl oste-otomilerle şekillendirilebilmesi, osseointegrasyona izin verebilmesi, minimal donör saha morbiditesi başlıca avantajlarıdır.
Abstract
Ameioblastoma is the most common epithelial odonto-genic tumor. Ameloblastomas usually occur in individuals aged 20-40 years; however, the unicystic variant most often occurs in adolescents. It may occur because of dental lamina, enamel organ, dental follicle and epithelial layer of the dentigerous cyst. The lesion is seen equally between males and females. The first symptom is usually painless bony expansion. This lesion occurs in both the maxilla and mandibula, but the posterior mandibula is the most common location; only 20% of lesions are found in the maxilla. Although ameioblastoma is generally not classified as a malignant lesion, it is extremely agressive and infiltrative. The treatment of ameioblastoma is surgical excision with wide free margin. All patients are examined radiologically during follow-up period. If excision is not sufficient, recurrence is common. Fibular osteocutaneous flap is one of the surgical technics preferred in mandibula reconstruction. Its main advantages are the following: Resection of straight bicortical bone up to 25 cm; possibility of multiple osteotomies and os-seointegration with minimal donor site morbidity.
