Hemiplejik hastalarda Bobath askısının fonksiyonel parametreler ve omuz subluksasyonu üzerine etkisi

Özet

Hemiplejik hastalarda üst ekstremite iyileşmesini olumsuz yönde etkileyen komplikasyonlar arasında gle-nohumeral eklemin aşağı subluksasyonu önemli yer tut­maktadır. Bu çalışmada hemiplejik hastalarda Bobath askısı kullanımının glenohumeral eklemin aşağı subluk­sasyonu ve fonksiyonel düzelme üzerine etkisi araştırıldı. Çalışmaya ortalama yaşı 58.7 olan toplam 32 hemiplejik hasta dahil edildi. Bu hastaların 12'si Bobath askısı kul­lanarak, 20'si askı kullanmaksızın 1 aylık çalışma süresi boyunca konvansiyonel rehabilitasyon programına tabi tutuldu. Her hastada, çalışma başında ve sonunda tonus, mobilite, omuz ağrısı, Brunnstrom'a göre motor evre ve glenohumeral eklemin subluksasyon derecesi tespit edil­di. Subluksasyon derecelendirmesi omuzun anterior oblik grafileri kullanılarak 5 kademede değerlendirildi. Bir aylık tedavi süresi sonunda, her iki grupta tonus, mobili­te, ağrı ve Brunnstrom motor evrelerindeki değişim ista­tistiksel olarak anlamlı bulunmadı. Çalışma sonunda askı kullanan 12 hastada subluksasyon derecesinin azalmış olduğu ya da aynı kaldığı tespit edildi. Askı kullanmayan grupta ise 13 hastada subluksasyon azalmış ya da aynı kalmış, 7 hastada ise artmış olarak bulundu. Bulguları­mız, Bobath askısı kullanımının hemiplejik hastalarda gle­nohumeral eklemde subluksasyon gelişmesini ve var o-lan subluksasyonun ilerlemesini önlemede yararlı oldu­ğunu göstermiştir.

Abstract

Downward subluxation of glenohumeral joint takes im­portant place among complications which affect upper ext­remity recovery negatively in hemiplegic patients. In this study the effect of Bobath sling on glenohumeral joint down­ward subluxation and functional recovery was investigated on hemiplegic patients. Total of 32 patients with hemiplegia with a mean age of 58.7 were included in this study. Twelve of these patients using Bobath sling and 20 of them without using Bobath sling participated conventional physiotherapy and rehabilitation program during one month study period. At the beginning and at the end of the study tonus, mobility, shoulder pain, motor stage accoording to Brunnstrom and grade of subluxation of glenohumeral joint were determined in each patient. Grading of shoulder subluxation was asse­ssed with 5 point categorization by using shoulder X-ray graphies in anterior oblique position. After one month the­rapy period changes in tonus, mobility, pain and Brunnst­rom motor stages were not found statistically significant in both groups. At the end of the study, in 12 patients using sling, the grade of subluxations were found to be decreased or the same. The difference was found statistically signifi­cant (p0.05). Our findings indi­cated that using Bobath sling is helpful to prevent develop­ment of subluxation and progression of existing subluxation in glenohumeral joint in hemiplegic patients.
