Özet
Hava yolu obstrüksiyonu, hipoventilasyon ve hipoksemi; genel anestezinin derlenme döneminde sık izlenen komplikasyonlardır. Pediyatrik hastalarda genel anestezi sonrası en sık gözlenen solunumsal komplikasyonlar, laringospazm ve laringeal ödemdir. Larinksin iritasyon ve travmaya maruz kalması, laringeal ödemin başlıca nedenidir. Yüksekten düşme nedeniyle hastanemize başvuran 8 yaşındaki erkek hastada sağ humerus lateral kondil kırığı tespit edilmesi üzerine operasyona karar verildi. Genel anestezi sırasında tekrarlayan entübasyon girişimi olan hastada, postoperatif birinci saatte inspiratuar stridor, öksürük, taşipne, dispne, huzursuzlukla karakterize laringeal ödem gelişti. Reanimasyon servisine alınan hasta, maske ile basınçlı oksijen, salbutamol nebül, budesonid nebül, i.v. deksametazon, aminofilin tedavisi ile komplikasyonsuz düzeldi. Biz de bu olgu nedeniyle çocuklarda postoperatif yakın takibin önemini tekrar vurgulamak istedik.
Abstract
The most common complications at recovery period are airway obstruction, hipoventilation and hipoxemia. Pediatric patients vulnerable to some postanesthetics complications which are laryngospasm and laryngeal edema. Laryngeal edema is associated with laryngeal irritation and trauma. An 8-year-old male patient was admitted to the emergency department of hospital because of falling. The operation was planned for fracture of lateral condyle of humerus. Intubation was performed after second attempt. Laryngeal edema which is characterized by cough, tachypnea, dyspnea, agitation and inspiratory stridor developed at the first postoperative hour. Inhalation of nebulized budesonid, salbutamol, I.V. dexamethasone, amynophyline and oxygen are used for treatment in intensive care unit. Treatment is successful without complication. The postanesthesia care unit are very important to observe the child to prevent severe complication.
