Importance of the test of thyroglobulin antibody for the diagnosis of autoimmune thyroiditis

Özet

Tiroglobülin Antikor Testinin Otoimmün Tiroidit Tanısındaki Önemi Otoimmün tiroidit (OT), hipotiroidinin toplumdaki en sık sebebidir ve özellikle orta ve ileri yaştaki kadınları etkiler. Tanı esas olarak anormal serum tirotropin konsantrasyonu ile birlikte serumda tiroid otoantikorlarının [tiroid peroksidaz (TPO) ve/veya tiroglobülin (Tg)] mevcudiyeti ile konulur. Her ne kadar OT tanısında ultrasonografi (US) istatistiksel olarak anlamlı bir role sahip olsa da (sensitivite=%80.70 ve spesifisite=%69.49, p=0.0001), günümüzde sadece tanıya yardımcı bir metot olarak kullanılmaktadır. OT tanısında otoantikorların yüksek titrelerde ve birlikte pozitiflikleri daha spesifikse de, vakaların önemli bir yüzdesinde otoantikorların tek başlarına pozitif oldukları görülmektedir. Ayrıca TPO antikorun tanıda kabul edilmiş daha yüksek sensitivitesi ve spesifisitesine rağmen, vakaların önemli bir yüzdesinde tek başına Tg otoantikor pozitifliği görülmektedir. Çalışmamızdaki 161 OT hastasının 138'inde sadece antiTPO antikor pozitifliği mevcuttu (sensitivite=%85.71), yani 23 vakada (% 14.28) OT tanısı sadece Tg antikor pozitifliği ile konuldu. Yardımcı bir tanı metodu olarak US dahi, bu TPO antikor negatif 23 OT hastasının sekizinde tanı açısından negatifti. Sonuç olarak, TPO antikorun OT tanısındaki yüksek sensitivitesi ve spesifisitesi ve US'nin istatistiksel olarak anlamlı rolüne rağmen Tg antikor, OT vakalarının önemli bir yüzdesinde hâlâ zorunlu bir tanı metodudur.

Abstract

Autoimmune thyroiditis (AT) is the most common cause of hypothyroidism in society, especially affecting the middle age and elder females. Its diagnosis is mainly dependent on the presence of thyroidal autoantibodies [thyroid peroxidase (TPO) and/or thyroglobulin (Tg)] together with an abnormal thyrotropin concentration in serum. Although the statistically significant role of thyroid ultrasonography (US) for the diagnosis of AT (sensitivity=80.70% and specificity=69.49%, p=0.0001), nowadays it is only used as a supporting tool for the diagnosis. Although higher titers and associated positivities of both of the autoantibodies are more specific for the diagnosis of AT, solitary positivities of them are present in an important percentage of cases. Additionally, although the accepted higher sensitivity and specificity of TPO Ab for the diagnosis, Tg Ab is positive alone in an important percentage of cases. In our study, TPO Ab has been positive in 138 cases among 161 cases with AT (sensitivity=85.71 %), so AT has been diagnosed by the positivity of Tg Ab alone in 23 cases (14.28%). Even US, as a supporting tool, has failed for the diagnosis of AT in eight of these TPO Ab negative 23 cases. As a result, although the higher sensitivity and specificity of the TPO Ab and statistical significance of US for the diagnosis of AT, Tg is a necessary laboratory test for the diagnosis of AT in an important percentage of cases.
