Pelvik kitlelerde malign-benign ayırımında serum CA 125 düzeyinin tanısal değeri

Özet
Pelvik kitle tespit edilen hastalarda malign-benign ayrımının yapılabilmesinde yüksek Serum CA 125 düzeylerinin etkinliğini belirlemek amaçlandı. Materyal-Metod: Dicle Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniğinde pelvik kitle nedeniyle öpere edilmiş ve Serum CA 125 düzeyleri çalışılmış toplam 111 hasta retrospektif olarak incelendi. Serum CA 125 düzeyi için cut-off değeri 35 İÜ/mi olarak kabul edildi. Hastalar histopatolojik tanılarına göre Grup 1; benign, Grup 2; malign, Grup 3; borderline olarak 3 gruba ayrıldı. Malign ve benign olgulardaki tespit edilmiş Serum CA 125 değerleri gruplar arasında karşılaştırıldı. İstatistiksel analizler için X2 ve Student t testi kullanıldı. Bulgular: Grup 1; benign (n=64), Grup 2; malign (n=45), Grup 3; borderline (n=2) tespit edildi. Grup 1 'de %29 (n=19), Grup 2'de %60 (n=27) hastada yüksek Serum CA 125 düzeyleri tespit edildi ve aralarındaki fark istatistik olarak anlamlı bulundu (p0.05). Sensitivite, spesifisite, pozitif ve negatif belirleyicilik sırasıyla menopozal durumdan bağımsız; %60, %70, %58 ve %71, premenopozal dönemde; %50, %68, %43 ve %74, postmenopozal dönemde; %73, %80, İ&87 ve %61 tespit edildi. Sonuç: Serum CA 125'in pelvik kitlelerin malign-be-ning ayırımında postmenopozal dönemde daha fazla olmak üzere belirgin olarak faydalı, ancak tek başına yetersiz olduğu kanısına varıldı.
Abstract
Aim: In this study, the effects of high serum CA 125 levels for the differentiation of malign and benign were investigated in patients with pelvic mass. Material-Method: A total of 111 patients undergone operation due to pelvic tumour in Obstetric and Gynaecology Clinic in Faculty of Medicine in University of Dicle, Turkey were retrospectively studied. The serum CA 125 levels of the patients were also included in the reports. The patients were divided into three groups according to histopathological findings as Group 1; benign, group 2; malign, group 3; borderline. The serum CA 125 values determined in the malign and benign cases were compared among groups. Statistical analysis were performed using X2 and Student t test.Results: The number of patients were assigned as Group 1; benign (n=64), Group 2; malign (n=45), Group 3 and borderline (n=2). High levels of serum CA 125 were determined in 29% (n=19) and 60% (n=27) of the patients in Group 1 and Group 2, respectively. The difference was statistically significant (p0.05). Sensitivity, specificity, positive and negative predictive value were determined 60%, 70%, 58% and 71% in patient with pre-postmenoposal status, 50%, 68%, 43% and 74% in patient with premenoposal status, 73%, 80%, 87% ve 61 % in patient with postmenoposal status, respectively. Conclusion: It is concluded that the serum CA 125 levels were found as useful criteria for the differentiation of malign and benign, in particular, in postmenoposal period but it is not sufficient as a sole diagnostic approach.
