Özet
Hipertansiyon (HT) teşhis ve tedavisi, kan basıncı (KB) çeşitli streslere bağlı olarak büyük ölçüde değişiklik gösterdiği için komplikedir. Özellikle doktor muayane odasında ölçümler sıklıkla daha yüksektir, bu durum beyaz gömlek hipertansiyonu (BGH) olarak adlandırılır. Toplumdaki yüksek prevalansı bilinmesine rağmen BGH'nun prognostik önemi halen tartışmalıdır. Metodlar: Rastgele olarak 50 yaş üzeri BGH ve nor-motansiyon (NT) hastalarını çalışmaya aldık. Fun-doskopik muayeneler hastaların teşhisini bilmeyen aynı oftalmolojist tarafından yapıldı. Fundoskopik bulguları belirlemek için Keith, Wagener ve Barker hipertansif retinopati (HR) klasifkasyonu kullanıldı. İstatistiksel analiz olarak oranların karşılaştırılması yöntemi kullanıldı. Sonuçlar: 56 BGH ve 51 NT vakasıçalışmaya alındı. Hiçbir grupta grade III ve IV HR olgusu gözlenmedi. BGH grubunda grade I HR(% 14.2) gözlenirken, bu oran NT grubunda %9.8(5/51) idi(p>0.05). Benzer olarak BGH grubunda 3 grade II HR (% 5.3) tesbit edildi, bu oran NT grubunda % 3.9(2/51) idi(p>0.05). Sonuçlar: BGH ve NT arasında fundoskopik bulgular arasındaki önemsiz fark ve BGH'nun toplumda, hatta genç yaşlarda bile, yaygın prevalansının bilin- mesine bağlı olarak, BGH'nunç eşitli streslere karşı bünyenin bir cevabı olduğu ve takibinin evde KB ölçümü ile sınırlı kalmasının uygun olacağını düşünüyoruz.
Abstract
Background: Diagnosis and management of hypertension (HT) is complicated by the fact that blood pressure (BP) varies greatly, depending on various stresses. In doctor's office in particular, measurements are often too high, which is defined as white coat hypertension (WCH). Although its known high prevalence in society, prognostic significance of WCH still remains controversial. Methods: We randomly took consecutive patients with WCH and sustained normotension (NT) over the age of 50 years. Funduscopic examination was performed by the same ophthalmologist who was blind about the patients' subclassification. Keith, Wagener, and Barker classification of hypertensive retinopathy (HR) was used to define funduscopic findings. Comparison of proportions was used asmethod of statistical analysis. Results: Fifty-six WCH and 51 sustained NT cases were studied. No case of grade III or IV HR was observed in neither group. Eight cases of grade I HR (14.2%) were detected in the WCH group, whereas this ratio was 9.8% (5/51) in the sustained NT group (p>0.05). Similarly, although three cases of grade II HR (5.3%) were detected in the WCH group, this ratio was 3.9% (2/51) in the sustained NT group (p>0.05). Conclusion: As a result of the insignificant differences between WCH and sustained NT according to the funduscopic findings and already known high prevalence of WCH in society, even in very early decades, we think that WCH is a response of body against various stresses and its follow-up should be limited to annual home BP measurements.
