Özet
Pnömatozis intestinalis, gastarointestinal sistemde gaz ile dolu kistik yapıların eşlik ettiği radyolojik ve klinik bulgudur. Daha çok pilor tıkanıklığına bağlı görülmektedir. Özefagustan rektuma kadar olan tüm gastrointestinal sistemi tutar. Rektal kanama, kiste bağlı volvuluıs ve tansiyon pneumoperitone-um gibi komplikasyonlar gelişmedikçe sadece sebep tedavi edilmelidir. Ameliyat öncesi safra kesesine penetran duodenal ülser olarak rapor edilen, fakat daha önce literatürde bildirilmemiş olan, intra, ekstra ve safra kesesinde serbest havanın eşlik ettiği pnömotasis intestinalise bağlı gelişen pnömo-peritonuemlu 44 yaşındaki erkek hasta sunulmuştur. Ameliyat sırasında pnömoperitoneum ve bir ince barsak segmentinde, içinde havanın bulunduğu multipl, yuvarlak yapılar görülerek pnömatozis intestinalis teşhisinden başka bir patoloji görülmedi. Hastaya yapılan özofagogastroduodenoskopide ve baryumlu özefagogastroduodenogramda pasaj problemine neden olmayan duodenal ülser olduğu görülerek hasta anti-ülser tedavisi ile taburcu edildi.
Abstract
Pnematosis intestinalis is a physical or radiological finding of cystic collection of gas within the wall of the gastrointestinal tract. It can be located anywhere along the gastro intestinal tract from the esophagus to the rectum, most frequently accompanied by pyloric stenosis. Ma-nagment is the treatment of the underlying conditions unless the complications are present, such as rectal bleeding, cyst-dependent volvulus or tension pneumo-peritoneum. We present a 44-year-old man with spontaneous pneumoperitoneum due to PI with free air in the intra and extrahepatic biliary system and also in the gallbladder, a coincidence not reported previously, with a diagnosis of duodenal ulcer penetrating the gallbladder. Pneumoperitoneum and multiple round structures over a segment of the small bowel were found during operation diagnosed as pneumotosis intestinalis without any other pathology. His esofagogastroduodenoscopy and barium filled esofagogastroduodenographies showed duodenal ulcer without any passage problems and ma-nuscripted anti-ulcer medication before discharged.
