Koah akut atakla pulmoner emboliyi belirlemede D-dimer düzeyinin rolü

Özet
KOAH tromboemboli vakalarının ayırıcı tanısında ve nedenleri arasında yer alır. KOAH akut atakta pulmoner emboliyi saptamak klinik tablo çok benzer olduğundan güçtür. Son yıllarda pulmoner emboli tanısında fibrin yıkım ürünlerinden biri olan D- dimer düzeyi gündeme gelmiştir. Çalışmamızda Ocak 2006- Ocak 2007 tarihleri arasında 100 KOAH Akut atak olgusuna D- dimer düzeyi bakarak pulmoner ebolinin ne sıklıkta akut atak nedeni olabilece- ğini belirlemeyi hedefledik. 100 olgunun 46 sında d- dimer düzeyi 500 ng/ml nin üzerinde bulundu. Bu olgularda klinik olasılıkta göz önünde bulundurularak V-P Sintigrafisi , EKO, Alt ekstremite venöz doplerUSG yapıldı. Toplam 10 olguda pulmoner emboli teflhis edildi. Özellikle KOAH akut atak nedeni belirlenemeyen olgularda basit ucuz ve kolay ulaşılabilir bir test olan d- dimer ölçümünün emboli olgularını saptamada etkili olacağı görülmüştür.
Abstract
Chronic obstructive lung disease (COPD) may have a role on the development of thromboembolic disease. It is clinically difficult to differentiate the exacerbation from pulmonary embolism (PE) in patients with COPD since the clinical features are similar. In recent years, a fibrin degradation product called d-dimer is evaluated for the diagnosis of thromboembolic disease. In this study we evaluated d-dimer levels in COPD exacerbation patients to determine the rate of PE among COPD patients. We tested 100 COPD patients who were hospitalized in our clinic between January 2006 and January 2007. Among 100 patients 46 had a d-dimer level above 500mg/ml. These patients were evaluated by ventilation-perfusion scanning, by echocardiography and by venous Doppler ultrasound test. Totally, 10 patients (21%) were diagnosed as pulmonary embolism. In conclusion, patients with exacerbation of COPD should also be tested for pulmonary embolism, and d-dimer is a simple and fast method to exclude the patients who have no thromboembolic disease.
