Göğüs hastalıkları referans hastanesindeki 61hemoptizili hastanın retrospektif incelenmesi

Özet
Çalışmamızda göğüs hastalıkları eğitim hastanesindeki kliniğimize hemoptizi şikayetiyle yatırılan 61 hastanın klinik radyolojik özellikleri, kullanılan tanı ve tedavi yöntemleri, hemoptizi etiyolojisi retrospektif olarak incelendi. Hastaların 47’si erkek(%77), 14’ü kadın(%23) ve yaş ortalaması 48.2 ± 17.6 yıldır.En sık hemoptizi nedeni 34( %55.6)olguyla akciğer tüberkülozu olarak bulundu,bu olguların 22’si( %36)aktif , 12’si (%19.6) ise inaktif-sekel akciğer tüberkülozudur.‹kinci sıklıkta 7 olguyla(% 11.4) pnömoni, üçüncü olarak da 5olguyla(%8.1) akciğer kanseri hemoptizi nedeni olarak saptandı.Hastalar hemoptizi miktarlarına göre 1)24saatte 200mlden az, 2)200-500ml, 3)500ml ve üzeri olmak üzere üç gruba ayrıldı; 2.grupta en fazla hemoptizi nedeni %14.6 ile (aktif ve inaktif) akciğer tüberkülozu olarak bulundu.Akciğ er grafisi bulgularında en sık asiner infiltrasyon %29.5 izlendi; 3 hastanın akciğer grafisi ve bilgisayarlı akciğer tomografisi normaldi.Balgamda aside dirençli basil tüm hastalarda bakıldı, %26.2 pozitif bulundu. Bronkoskopi yapılan 30 (49.1%) hastamızın %73.3’ünde endobronşiyal lezyon saptanmadı; %16.6’sında aktif kanama odağı,%3.3’ünde lob bronşunda pıhtı, %6.6’sında kitle görüldü.Dört (%6.5) hastamıza 2 kistektomi, 1 lobektomi, 1 pnömonektomi olmak üzere cerrahi girişim uygulandı.Üç(%4.9) hastamızda hemoptizi nedeni belirlenemedi, bunlardan biri abondan hemoptiziyle kaybedildi, diğer 2 hasta iki yıldır semptomsuz takiptedir.Sonuçta, bizim hasta grubumuzda en sık hemoptizi nedeni akciğer tüberkülozu olarak bulundu bu nedenle tanı ve tedavi yaklaşımında balgam ARB incelemesi önemli bir yere sahiptir.Hemoptizi nedeni olarak tüberküloz oranının yüksek bulunmasının da ülkemizdeki prevalans ve hastanemizin en fazla tüberküloz yatağına sahip göğüs hastalıkları merkezi olmasıyla ilişkili olduğunu düşünmekteyiz.
Abstract
In our study, the clinical and radiological features of 61 patients who admitted to our clinic for hemoptysis in a reference hospital for chest diseases, the diagnostic and thearapeutic tools and etiology for hemoptysis were investigated retrospectively.Of the patients, 47(77%) were men and 1423(%) women ,and the mean age was 48.2 ± 17.6.The most common cause of hemoptysis was pulmonary tuberculosis, of these cases, 22(36%) were active and 12(19.6%) inactive tuberculosis.The second cause was pneumonia(11.4%, 7 patients) and the third one was lung cancer (8.1%, 5patients) Three groups were consisted according to amount of hemoptysis; 1) less than 200ml 2) 200-500ml 3) much more than 500ml per 24 hour; the most common cause of hemoptysis was pulmonary tuberculosis (14.6%,active and inaktive) in group 2.The most common finding( 29.5%) in chest radiographs was acinar infiltration; three patients’ chest radiographs and computed lung tomography scans were normal.Sputum mear examination for acid fast bacilli(ARB) was performed in all patients and were positive in 26.2% of them.Fiberoptic bronchoscopy was performed in 30 (49.1%) patients and seen active bleeding site in 16.6%, coagulum in the lobar bronchus in 3.3%, endobronchial mass in 6.6%of them; there were no endobronchial lesions in 73.3% of them .The surgical interventions(2 cystectomy,1 lobectomy, 1 pneumonectomy) were performed in 4(6.5%) patients.The causes of hemoptysis were unknown in 3(%4.9) patients, one of them was died with massive hemoptysis and 2 patients were followed for two years without symptoms.In conclusion; the most frequent cause of hemoptysis was pulmonary tuberculosis in our patients. So sputum smear examination ARB is important for diagnostic and therapeutic approach.We think that the high rate of tuberculosis in the causes of our hemoptysis cases is probably due to prevalence of tuberculosis in our country and due to speciality of our hospital is the center for chest disease that had the maximum bed capacity for patients with tuberculosis patients .
