Özet
Malign tumorlerde, tumoru drene eden bolgesel lenf nodlarında nonkazeoz epiteloid hucre granulomları gorulebilir. Sistemik sarkoidoza ait bir bulgu saptanmadıı takdirde bu patolojik deişiklikler sarkoid reaksiyon olarak adlandırılır. En sık skuamoz hucreli akcier kanseri ile birlikte gorulur. Kucuk hucreli akcier kanseri ile birlikteli i daha nadirdir. 56 yaşında erkek hasta toraks bilgisayarlı tomografisinde saptanan sa ust lobda kitle lezyon nedeni ile kliniimize tetkik amaclı yatırıldı. Fiberoptik bronkoskopisinde endobronşial lezyon gorulmedi ve tanı amaclı transtorasik ince ine aspirasyon biopsisi (TT‹- AB) yapıldı. Patolojisi malign epitelyal tumor hucreleri olarak rapor edildi. işlem esnasında pnomotoraks gelişen hastaya tup torakostomi ve kapalı su altı drenajı uygulandı. Ardından hastaya tanı amaclı mediastinoskopi yapıldı ve histopatolojik inceleme sonucu epiteloid histiositler ve langhans dev hucreleri iceren nonkazeoz tuberkuller olarak deerlendirildi. Bunun uzerine hastaya ikinci kez yapılan TT‹AB sonucu kucuk hucreli akcier kanseri olarak rapor edildi. Sistemik sarkoidoz bulgusu saptanmayan hastada mediastinal lenf nodlarındaki de- işiklikler sarkoid reaksiyon olarak deerlendirildi. Biz de nadir gorulmesi ve malign tumorlerle sarkoid reaksiyon arasındaki ilişkiyi gostermek amacı ile bu vakayı sunduk.
Abstract
In malignant tumors non-caseous epiteloid cell granulomas infiltrating regional lymph nodes are seen seldomly. These pathologic changes are called sarcoid reactions if systemic manisfestations of sarcoidosis is not present. Although they are mostly seen in squamous cell carcinoma of the lung, it’s rarely seen in small cell carsinoma. In this case report A 56-year-old male patient was admitted to our hospital for investigation of a mass lesion in computed tomography. We didn’t see any endobronchial lesion in fiberoptik bronchoscopy. Transthoracic fine needle aspiration biopsy (TTFNB) for diagnosis was performed. The pathological diagnosis was malignant epithelial tumor cells. Iatrogenic pneumothorax was developed during procedure and closed seal-underwater drainage was applied. Diagnostic mediastinoscopy was performed afterwards. Histopathological investigation of the specimen showed noncaseous tubercules including langhans giant cells consisting of epiteloid histiocytes. TTFNB was repeated and small cell carcinoma was diagnosed. Pathological changes of the lymph nodes was evaluated as sarcoid reactions because there was no clinical evidence of systemic sarcoidosis. In this case report we aimed to point out the relationship of malignancy and sarcoid reaction of lymph nodes and rare coincidence of small cell carcinoma with sarcoid reaction.
