Pulmoner tromboemboli olgularının uzun dönem takip sonuçları

Özet
Pulmoner tromboemboli olgularında izlem, tanı ve tedavi ile ilgili çalışmaların çokluğuna karşın bu olguların uzun dönem takip sonuçları ile ilgili bilgilerimiz daha azdır. Çalışmamızda Süreyyapaşa Göğüs Hastalıkları ve Göğüs Cerrahisi Eğitim ve Araştırma Hastanesi, Göğüs 3 Kliniğinde, 2000-2005 yılları arasında pulmoner tromboemboli (PTE) tanısı almış 77 hastadan kontrole gelen ve/veya telefonla ulaşılarak sorgulanan 35 olgunun ilk tanı anındaki sonuçları ile uzun dönem takip sonuçlarını değerlendirmeyi amaçladık (K/E: 8/27, yaş ortalaması: 51,6±13,6). Hastaların predispozan faktörleri, tanı kriterleri, tedavi süresi, tedavi sonundaki rezolüsyon durumları ve prognozları değerlendirildi. Hastaların ortalama izlem süresi 24.6±18.4 ay idi. İlk tanı anında %28.6 hastada herhangi bir risk faktörü saptanmazken, %11.4 olguda travma/cerrahi, %8.6 olguda kardiyak hastalık, %31.4 olguda derin ven trombozu saptandı. Tedavinin 6. ayında 21 hastaya alt ekstremite venöz doppler ultrasonografi tekrarlandı, 15 olguda trombüs saptanmadı, 6 olguda trombüsün devam etmesi üzerine tedavileri 1 yıla uzatıldı. 1. yılın sonunda tedavileri kesilen bu olguların ikisinde halen kardiyak şikâyetler mevcuttu. Sadece ileri yaştaki bir hastada, tedavinin 1 .yılında rekürrens görüldü. Ölen 3 olgunun hepsinde birden çok predispozan faktör mevcuttu (ileri yaş (n= 2), kronik obstrüktif akciğer hastalığı (n=1), kanser (n=3) (2 olgu adenokarsinom, 1 malign perikardial effüzyon)). Otuz beş olgu birlikte değerlendirildiğinde halen 18 olgu (% 51,4) semptom-suz olup 11 olgunun PE ile ilişkisiz şikâyetleri vardı.Sonuç olarak, PTE olgularının izleminde ileri yaş, altta yatan kalp hastalığı ve malignite gibi ilk muayenede saptanacak predispozan faktörlerin, uzun dönem prognoz hakkında fikir vermesi açısından önemi unutulmamalıdır.
Abstract
The extensive literature on pulmonary thromboembo-lic disease deals mainly with its follow up, diagnosis and early treatment. There are however only a few long term studies. We aimed to evaluate long term follow-up of patients with pulmonary thromboembolism (PTE) who were diagnosed in Süreyyapaşa Training and Research Hospital, Third Clinic, in 2000-2005. Thirty five patients who were contacted by telephone and reevaluated of 77 patients with PE were included in this study (F/M:8/27, mean age: 51,6±13,6). We reviewed predisposing factors, diagnostic criterias, treatment-duration, resolution after treatment and prognosis. Patients were followed-up for mean 24,65±18,48 months. 28,6% patients had no risk factors, 11,4% patients had trauma/surgery, 8,6% had cardiac disease, 31,4% had deep venous thrombosis in initial diagnosis. Lower extremities doppler ultraso-nography was reperformed at sixth month of treatment to 21 patients. Deep venous thrombosis was not detected in 15 patients and detected in 6 patients.Therefore.the treatment was continued to one year in these 6 cases. Cardiac problems had been continued in two of patients whose treatment was stopped at the end of one year. Recurrence was seen in only one old patient within one year of treatment. 3 patients who died had more than one predisposing facors. The conditions associated with these deaths were age (n=2), chronic obstructive pulmonary disease (n=1), cancer (n=1 definite diagnosis, n=2 suspected diagnosis) and tuberculous pleurisy (n=1). When 35 cases evaluated, 18 patients (51,4%) have still symptom free and 11 patients have symptoms that are not associated with PE. In conclusion, the importance of predisposing factors like age, cardiac disease and malignancy which can detected at first examination could not be forgotten that they can help estimating the long term prognosis of PTE.
