Özet
Kolesistektomi sindirim sisteminde en çok yapılan ameliyatlardan biridir.Yıllardan beri yapılan açık kolesistektomi kolelityazın tehlikesiz, güvenilir ve etkili bir tedavisidir. Morbiditesi az, mortalitesi ise % 1’in altındadır. Minimal invaziv cerrahinin önem kazandığı şu dönemde laparoskopik kolesistektomi açık kolesistektominin önüne geçmiştir . Hastanede kalış süresinin kısalığı, kozmetik sebepler ve erken iş başı yapma gibi avantajları laparoskopik kolesistektomiyi öne çıkarmıştır.Laparoskopik kolesistektominin gördüğü hızlı ve yaygın kabule karşın komplikasyon oranı açık kolesistektomiden yüksektir. Safra yollarının varyasyonları çok sık görülür. Bu nedenle anatomik yapıları ortaya koymada zorlukla karşılaşıldığında açık kolesistektomiye geçmekte tereddüt edilmemelidir. Biz de bu yazı ile dev safra kesesi taşı nedeni ile la-aroskopik başlayıp, gelişebilecek komplikasyonlar açısından laparoskopik kolesistektomide ısrarcı olmayıp açık kolesistektomiye geçtiğimiz bir olguyu sunmayı amaçladık. 1. Abstract
Cholecystectomy is one of the most frequently performed operations of the alimentary tract. Open cholecystectomy which has been practised since many years,is a safe and effective treatment modality for cholelithiasis. It has a low morbidity rate, with a mortality rate of less than 1 percent. In these glorious days of minimal invasive surgery; laparoscopic cholecystectomy has dominated open cholecystectomy. It has many advantages such as; shorter hospitalisation period, cosmetic reasons and early return to work. Laparoscopic cholecystectomy has gained a fast and wide acceptance but on the contrary; it has a higher rate of complication. Variations of the hepatic and cholesistic vessels and bile canals are quite frequent. So when it gets hard to understand the anatomy, the surgeon should not hesitate to swift to open cholecystectomy.With this piece of literature; we tried to summarize a case of huge gallbladder stone operation; during which we swifted to open cholecystectomy, because of the possible complications.
