Özet

Bu çalışmada, romatoid artritli hastalarda akciğer tutulumunun varlığı araştırılmıştır. Çalışmaya romatoid artrit (RA) tanısı bulunan 40 erişkin hasta (37 kadın, 3 erkek; yaş ortaması 52.6±8.62) dahil edildi. Hastaların yaş, cinsiyet, hastalık süresi, sigara kullanma alışkanlığı, tedavi protokolü, geçirilmiş akciğer hastalığı öykü- sü bilgileri kaydedildi. PA akciğer grafisi, solunum fonksiyon testleri (SFT), yüksek rezolüsyonlu bilgisayarlı tomografi (YRBT) tetkikleri yapıldı. PA akciğer grafisi, YRBT, SFT sırasıyla %19.4, %33.3, %50 oranında anormallik gösterdi. YRBT ile en sık konulan tanı interstisyel akciğer hastalığıydı (%35). YRBT anor- malliği ile yaş, cinsiyet, hastalık süresi, sigara kullanma alışkanlığı, tedavi protokolü, geçirilmiş akciğer hastalığı öyküsü arasında anlamlı ilişki saptanmadı. Anormal YRBT bulguları ile düşük DLCO/VA değerleri arasında anlamlı ilişki saptandı. Sonuç olarak RA’de akciğer tutulumunu değerlendirmede YRBT, hem PA akciğer grafisinden, hem de SFT’den üstün bulundu.

Abstract

In this study, the presence of pulmonary involvement in patients with rheumatoid arthritis (RA) was investigated. Forty adults patients with RA (37 women, 3 men; mean age 52.6±8.62) were included in the study. Patients age, gender, duration of the disease, smoking habits, treatment protocol, previous history of lung disease were recorded. PA chest radiography, pulmonary function tests (PFT), high resolution computed tomography (HRCT) were performed. PA chest radiography, PFT and HRCT findings showed 19.4%, 33.3%, 50% abnormalities, respectively. Interstitial lung disease was the most common finding on HRCT (35%). There was no statistically significant correlation between HRCT abnormality and age, gender, duration of the disease, smoking status, treatment protocol and previous history of lung disease. There was statistically significant correlation between HRCT abnormality and low DLCO/VA values. As a result, HRCT have higher sensitivity for assessing pulmonary involvement in RA from both PA chest radiography and PFT.
