Özet

Osteoporoz; kemik kitlesinde azalmaya bağlı olarak, kemiğin zayıflaması, özellikle vertebra, kalça ve el bileğinde kırık riskinde artışa yol açan bir iskelet sistemi hastalığıdır. Osteoporoz tanısı sıklıkla postmenopozal veya senil osteoporoza indirgenmiş olup, kemik mineral yoğunluğu (KMY) düşük olan hastalar primer osteoporoz tanısı ile tedavi edilmektedir. Bu durum sekonder osteoporoz nedenlerinin atlanmasına veya tanısının konulmasında gecikmelere neden olmaktadır. Altı ay önce bel ağrısı şikayetiyle özel bir hastaneye başvuran, yapılan Dual Enerji X-ray Absorpsiyometre (DEXA) ölçümü sonrası osteoporoz tanısı konularak bifosfonat başlanan ancak iç hastalıkları kliniğimizde multipl miyelom tanısı konulan bir olguyu sunmak istedik.

Abstract

Osteoporosis; the weakening of the bone due to a decrease in bone mass, is a skeletal disease that increase the risk of fractures especially in the spine, hip and wrist. Diagnosis of osteoporosis often is reduced in postmenopausal or senile osteoporosis, in patients with low bone mineral density (BMD) therapy is started with the diagnosis of primary osteoporosis. This situation leads to delay the diagnosis of secondary osteoporosis or causes to be bypassed. We wanted to present a patient who applied to a private hospital with low back pain 6 mount ago and according to the dual energy Xray Absorptiometry (DEXA) measurement diagnosed as osteoporoses and bisphosphonate treatment was started and then diagnosed as multiple myeloma in our internal medicine department.
