Odematoz seyirli Henoch-Schönlein purpuralı olguların değerlendirilmesi

Özet

Henoch Schonlein purpurası(HSP.), birden çoksistemi etkileyebilen ,öncelikle deri, gastrointestinal sistem,eklem ve böbreğin küçük çaplı kandamarlarını ,özellikle de postkapiller venülleri tutan çocukluk çağının en sık görülen vaskülitidir.En sık 2-8 yaş arasında görülür. Erkek/kız oranı1,5 dır. Sistemik bulgular hastaların %80 inde ortaya çıkar ve en sık bulgu purpurik deri döküntüsüdür. Diğer sık görülen bulgular artrit,böbrek tutulumu ve gastrointestinal problemlerdir.HSPvaskülitine bağlı yumuşak doku ödemi insidansılitaratürde %4-50 oranlarında bildirilmektedir.(1)Biz bu çalışmada yaşları 2,5 ile 11 arasında değişen scalp ,testis,sırt,bel ve ayak bileği bölgelerinde ödemlerle gelen 6 çocuk hastamızı bildirdik. Bir hastamız testis torsiyonu ile opere oldu.Hastalarımıza doku ve skrotal ödem varlığındakullanılan prednizolonun semptomları rahatlatarak tedavideki yerine dikkat çektik. Özellikle,ödamatöz seyreden Henoch Schonlein olgularımızı sunarak, litaratür ışığında hastalığı tekrargözden geçirdik.

Abstract

Henoch Schönlein purpura(HSP) , is the mostcommonly seen vaskulites of childhood, primarily affects postcapillary venules of dermis, ,gastrointestinal tract , joint and kidney. It is mainlyseen in children between 2 and 8 years with amale to famale ratio of 1,5 . Systemic symptomscan be presented in %80 of patients and themost commonly seen symptom is purpuric rash.Arthritis , renal involvement and gastrointestinaldisorders are the other manifestations. The incidence of soft tissue edema correlated with HSPis reported in %4-50 of cases in literatureIn this study, we reported 6 children whose agesare between 2,5 and 11 years, presented to ourclinic with scalp ,scrotal, perineal and ankle swelling. Our 1 patient was operated with testiculartorsion. We showed that in the presence of softtissue swelling ,using prednizolon therapy canrelieve the symptoms and discomfort. When wereported this edematous course of HenochSchönlein purpura cases ,we evaluated the disease again in the light of the literature
