Basil pozitif tüberküloz olgularının tedavi sonuçlarının değerlendirilmesi

Özet
Amaç: Tüberküloz, uzun süre tedavi gerektirenkronik bir hastalıktır ve tüm dünyada önemli birhalk sağlığı sorunu olmayı sürdürmektedir. Tüberküloz basilinin kaynağı hasta kişilerdir. Basilinsolunum yoluyla bulaşması nedeniyle hastalığınkontrolü zordur. Tüberkülozun kontrolündeönemli olan, hastalığın erken teşhis edilmesi, tanı konulan olguların tedavi edilmeleri ve kür sağlanmasıdır. Çalışmamızda merkezimize başvuran basil pozitif olguların tedavi sonuçlarını değerlendirmeyi amaçladık.Gereç ve Yöntem: Çalışmamıza fiubat 2002 -Mayıs 2003 tarihleri arasında Süreyyapaşa Göğüs Hastalıkları ve Göğüs Cerrahisi Eğitim veAraştırma Hastanesine tüberküloz tanısı ile yatırılan, balgamda ARB (+) olan 145 hasta dahiledildi. Her hasta için demografik veriler ve 6 aylık takipte hastalığın klinik, radyolojik ve bakteriyolojik gidişini gösteren takip formları dolduruldu.Daha sonra hastalar ilgili servislerin takip ve tedavilerine bırakılarak tedavilerinin 2. ve 6. aylarında yeniden görüldü. Tedavi sonuçlarının raporlanmasında ortak bir terminoloji kullanmakadına WHO tarafından belirlenen 6 temel tedavisonuçları kullanıldı.Bulgular: Hastaların 99u erkek (%68,3), 46sıkadındı (%31,7). Yaş ortalaması 35,6 ± 13,7 (1572) idi. Hastaların %64.1i (n=93) evli,%35,9u (n=52) ise bekar yada dul idi. Eğitim düzeylerine bakıldığında, büyük bir bölümü ilkokulmezunu idi. Hastaların %49u (n=71) düşük gelirli olup %11.7sinde (n=17) eski tüberküloz öyküsü vardı. Hastalardan birinin tedavisinin üçüncüayında iken septik şok ile ex olduğu saptandı.Onun için tedavi sonuçlarının değerlendirilmesinde çalışma dışı bırakıldı. Hastaların % 70,1i(n=101/144) tedavilerini merkezimizde tamamladılar. Bu hastaların % 42,7sinde (n=68/144) mikrobiyolojik kür sağlanırken % 22,9 (n=33/144)hasta tedavilerini tamamladılar. Merkezimizdetakip edilen hastalarda tedavi başarısı % 70,1(n=101/144) olarak saptandı. % 7,6 (n=11/144)hastanın tedavilerini terk ettikleri görüldü. Nakilgiden 23 hastaya telefonla ulaşıldığında tadavilerini tamamladıkları öğrenildi. Bu hastalar da dikkate alındığında merkezimizde yapılan bu çalışmada tedavi başarısı % 86,1 (n=124/144) olaraksaptandı.Sonuç: Çalışmamızda hedeflenen tedavi başarısı yakalanmış olmakla birlikte özellikle riskli gruplarda doğrudan gözetimli tedavi uygulaması iledaha başarılı sonuçlar alınacağı kanısındayız.Anahtar kelimeler: Tüberküloz, tedavi sonuçları
Abstract
Tuberculosis is a chronic disease that re-quires long-term treatment and continues to bean important health problem throughout theworld. The source of tuberculosis bacillus is indi-viduals with the disease. It is difficult to controlthe disease since it is transmitted through respi-ration. Important issues in controlling tuberculo-sis are early diagnosis of the disease, treatmentof the diagnosed cases and providing cures. Inour study we aimed to evaluate the treatment results of bacillus positive cases that had appliedto our center.Metod: Atotal of 145 new patients admitted toSüreyyapaşa Chest Diseases and Chest Surgery Training and Research Hospital betweenFebruary 2002-May 2003 with the diagnosis oftuberculosis and having ARB (+) in the sputumwere included in the study. Demographical dataand follow-up forms showing the clinical, radiological and bacteriological course of the diseasewithin six months of follow-up were filled out foreach patient. Then, patients were left to the follow-up and treatment of related services and were seen again during the second and six monthsof their treatment. In order to use a common terminology in the reporting of treatment results, sixbasic treatment results specified by WHO wereused.Results: Atotal of 99 of the patients were male(68.3%) and 46 of the patients were (31.7%) female. Age average was 35.6 ± 13.7 (15-72). Atotal of 64.1% of the patients (n=93) were married,35.9% (n=52) were single or widowed. Wheneducational levels were checked, most patientshad a primary school education level. Atotal of49% (n=71) of the patients were from low-income families and 11.7% (n=17) had a previoushistory of tuberculosis. One patient was determined to be ex due to septic shock during the thirdmonth of his/her treatment. Therefore he/shewas exclu- ded from the study during the evaluation of treatment results. Atotal of 70.1%(n=101/144) of the patients completed their treatment in our center. Amicrobiological cure was 3 provided for 42.7 (n=68/144) of these patients. Atotal of 22.9% (n=33/144) of the patients completed their treatment. The treatment success of thepatients who were followed-up in our center wasdetermined to be 70.1% (n=101/144). Atotal of7.6% (n=11/144) of the patients abandoned theirtreatment. When 23 patients who were transferred to other centers were contacted by telephone, it was understood that they had completedtheir treatment. When these patients are alsoconsidered, the treatment success of this studyconducted in our center was determined to be86.1% (n=124/144).Conclusion: As a result, though we have reached the targeted treatment success rate in ourstudy, we believe that more successful resultscan be obtained with the application of directlyobserved therapy, particularly in risky groups.
